Description
Overview of the medicine
Ciprofloxacin is a fluoroquinolone antibiotic used to treat various bacterial infections, including urinary tract infections, respiratory tract infections, skin and soft tissue infections, and bone and joint infections.
Uses & Indications
Dosage
Adults
Typically 250 mg to 750 mg orally twice daily, depending on the type and severity of infection.
Elderly
Dosage should be adjusted based on renal function. No specific dose reduction is needed solely for age.
Renal_impairment
For creatinine clearance (CrCl) between 30-50 mL/min, 250-500 mg every 12 hours. For CrCl < 30 mL/min, 250-500 mg every 18-24 hours.
How to Take
Administer orally with or without food. Do not take with dairy products (e.g., milk, yogurt) or calcium-fortified juices alone, as these can significantly decrease absorption. Take with plenty of fluids.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes involved in bacterial DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.

Side Effects
Contraindications
- Hypersensitivity to ciprofloxacin or other fluoroquinolones.
- Co-administration with Tizanidine.
- History of tendon disorders associated with quinolone use.
- Myasthenia gravis exacerbation.
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, increasing bleeding risk.
Tizanidine
Concomitant use is contraindicated due to increased tizanidine plasma concentrations, which can lead to hypotension and sedation.
Cyclosporine
May increase serum cyclosporine levels.
Theophylline
Increases serum concentrations of theophylline, potentially leading to toxicity.
Antacids/Iron/Zinc
Reduces absorption of ciprofloxacin; administer ciprofloxacin 2 hours before or 6 hours after these agents.

In case of acute overdose, gastric lavage may be considered. Maintain adequate hydration. Symptomatic and supportive treatment is recommended. Hemodialysis or peritoneal dialysis remove only a small amount of ciprofloxacin from the body.

Pregnancy Category C. Ciprofloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted into breast milk and therefore not recommended for use during lactation due to potential risks to the infant (e.g., arthropathy).
