Uses & Indications
Dosage
Adults
For Vitamin D deficiency prophylaxis: 2000 IU once daily. For Vitamin D deficiency treatment: Doses may vary based on severity, often 2000 IU to 4000 IU daily or higher doses weekly/monthly, as advised by a physician.
Elderly
Similar to adults. May require closer monitoring of Vitamin D levels due to potential for reduced absorption or synthesis.
Renal_impairment
Use with caution. Monitoring of serum calcium and phosphate levels is important. In severe renal impairment, active vitamin D metabolites may be preferred.
How to Take
Take the capsule orally with water, preferably with a meal containing fat to enhance absorption. Do not chew or crush the capsule.
Mechanism of Action
Cholecalciferol (Vitamin D3) is hydroxylated in the liver to 25-hydroxycholecalciferol (calcifediol), which is then further hydroxylated in the kidneys to 1,25-dihydroxycholecalciferol (calcitriol). Calcitriol is the active form of Vitamin D, which regulates calcium and phosphate homeostasis by promoting absorption from the gut, reabsorption in the kidneys, and mobilization from bone.
Pharmacokinetics
Onset
Therapeutic effects may take several days to weeks to manifest, as stores need to be replenished and active metabolites formed.
Excretion
Excreted primarily in bile and feces, with only a small amount excreted in urine.
Half life
The elimination half-life of cholecalciferol itself is short (approx. 24 hours), but the active metabolite calcifediol (25-OH-D3) has a half-life of 2-3 weeks, and calcitriol (1,25-(OH)2D3) has a half-life of 4-6 hours.
Absorption
Well absorbed from the gastrointestinal tract, especially in the presence of bile salts and dietary fat. Peak plasma concentrations are reached within 4-5 hours.

Drug Interactions

Storage

Overdose
Symptoms of overdose include hypercalcemia (nausea, vomiting, constipation, polyuria, polydipsia, weakness, fatigue, headache, metallic taste). In severe cases, renal damage, cardiac arrhythmias, and coma can occur. Treatment involves discontinuing Vitamin D, low calcium diet, hydration, and medical management of hypercalcemia.
Pregnancy & Lactation
Vitamin D is essential during pregnancy and lactation. However, high doses should be avoided. Consult your doctor to ensure appropriate dosage to prevent deficiency without exceeding recommended limits.
