D-Balance 40000 IU Capsule is a high-strength vitamin D3 supplement used to treat and prevent vitamin D deficiency, promote bone health, and support overall calcium and phosphate balance in the body.
Uses & Indications
Dosage
Adults
For severe Vitamin D deficiency: 40,000 IU once weekly for 7-10 weeks, then maintenance dose. Or as directed by physician based on serum 25(OH)D levels.
Elderly
Similar to adult dosage, but caution should be exercised and calcium levels monitored closely due to potential for renal impairment.
Renal_impairment
Use with caution. Dosage adjustments may be necessary, and calcium, phosphate, and renal function must be monitored regularly.
How to Take
For oral administration. The capsule should be swallowed whole with water, preferably with the largest meal of the day to enhance absorption.
Mechanism of Action
Cholecalciferol (Vitamin D3) is converted in the liver and kidneys to its active form, calcitriol. Calcitriol regulates calcium and phosphate homeostasis by promoting absorption from the gut, reabsorption in the kidneys, and bone mineralization.

Onset
Effects on serum calcium levels can be observed within days to weeks; full therapeutic effect may take several weeks.
Excretion
Primarily excreted via bile into feces; a small amount is excreted in urine.
Half life
The elimination half-life of parent cholecalciferol is approximately 24 hours. The active metabolite, 25-hydroxyvitamin D, has a half-life of about 15 days.
Absorption
Well absorbed from the small intestine in the presence of bile salts; absorption is enhanced by fatty meals.
Metabolism
Hydroxylated in the liver to 25-hydroxyvitamin D (calcifediol) and then in the kidneys to 1,25-dihydroxyvitamin D (calcitriol), the biologically active form.
Side Effects
Contraindications
- •Hypercalcaemia (high levels of calcium in the blood)
- •Hypervitaminosis D (excessive amounts of Vitamin D in the body)
- •Nephrolithiasis (kidney stones) or nephrocalcinosis (calcium deposits in the kidneys)
- •Known hypersensitivity to cholecalciferol or any of the excipients
Drug Interactions
Corticosteroids
May reduce the effects of vitamin D by impairing intestinal calcium absorption.
Thiazide diuretics
May reduce urinary calcium excretion and increase the risk of hypercalcaemia.
Cholestyramine, mineral oil, orlistat
May impair the absorption of fat-soluble vitamins, including vitamin D.
Anticonvulsants (e.g., phenytoin, barbiturates)
May increase the metabolism of vitamin D, potentially reducing its effectiveness.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose (hypervitaminosis D leading to hypercalcaemia) include nausea, vomiting, loss of appetite, constipation, weakness, fatigue, confusion, excessive thirst, increased urination, and kidney stones. Treatment involves discontinuing vitamin D, a low-calcium diet, and adequate hydration.
Pregnancy & Lactation
Vitamin D is essential during pregnancy and lactation. However, high doses (above recommended daily intake) should only be used under medical supervision, with careful monitoring of maternal and fetal/infant calcium levels.
