Description
Overview of the medicine
d-balance 20000 IU capsule is a high-potency Vitamin D3 supplement used to treat and prevent Vitamin D deficiency, supporting bone health, immune function, and overall well-being.
Uses & Indications
Dosage
Adults
For severe deficiency, 20000 IU once weekly for 7-10 weeks, followed by maintenance dose as advised by a physician. For maintenance, 20000 IU every 2-4 weeks. Always consult a physician.
Elderly
Similar to adults, but careful monitoring of serum calcium and renal function is recommended due to potential age-related changes.
Renal_impairment
Use with caution. Monitor serum calcium, phosphate, and parathyroid hormone levels. May require active vitamin D metabolites instead of cholecalciferol.
How to Take
Take orally, preferably with the largest meal of the day to enhance absorption, as Vitamin D is a fat-soluble vitamin.
Mechanism of Action
Cholecalciferol (Vitamin D3) is converted in the liver and kidneys to its active form, calcitriol. Calcitriol regulates calcium and phosphate homeostasis by increasing intestinal absorption, promoting renal reabsorption, and influencing bone mineralization.
Pharmacokinetics
Onset
Therapeutic effects are generally slow, requiring days to weeks for significant changes in Vitamin D levels.
Excretion
Primarily excreted in the bile and feces, with a smaller portion excreted in the urine.
Half life
The half-life of cholecalciferol in serum is approximately 24 hours, but the active metabolite 25-hydroxyvitamin D (calcifediol) has a half-life of 2-3 weeks.
Absorption
Well absorbed from the small intestine, primarily in the presence of bile salts and dietary fat. Peak plasma concentrations are reached within 4-5 hours.
Metabolism
Initially hydroxylated in the liver to 25-hydroxyvitamin D (calcifediol), then further hydroxylated in the kidneys to 1,25-dihydroxyvitamin D (calcitriol), the biologically active form.
Side Effects
Contraindications
- •Hypercalcemia (high calcium levels in the blood).
- •Hypervitaminosis D (excessive levels of Vitamin D).
- •Malabsorption syndromes (unless specifically managed).
- •Hypersensitivity to cholecalciferol or any components of the capsule.
Drug Interactions
Corticosteroids
Can reduce the effects of Vitamin D.
Cardiac glycosides
Hypercalcemia caused by Vitamin D overdose may potentiate the effects of cardiac glycosides, increasing the risk of arrhythmias.
Thiazide diuretics
May increase the risk of hypercalcemia.
Anticonvulsants (e.g., phenytoin, barbiturates)
May increase the metabolism of Vitamin D, leading to reduced efficacy.
Bile acid sequestrants (e.g., cholestyramine), mineral oil, orlistat
Can impair the absorption of Vitamin D.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia (nausea, vomiting, loss of appetite, thirst, polyuria, constipation, muscle weakness, fatigue, headache, confusion). Severe cases can lead to renal impairment and soft tissue calcification. Treatment involves discontinuing Vitamin D, hydration, loop diuretics, and corticosteroids as needed.
Pregnancy & Lactation
Vitamin D is an essential nutrient during pregnancy and lactation. High doses (>4000 IU/day) should be taken under medical supervision. It is excreted in small amounts in breast milk and is generally considered safe and beneficial for both mother and infant.
