Description
Overview of the medicine
D-Balance 50,000 IU Capsule is a high-potency Vitamin D3 (Cholecalciferol) supplement used to treat and prevent vitamin D deficiency. It helps the body absorb calcium and phosphorus, crucial for maintaining strong bones and teeth.
Uses & Indications
Dosage
Adults
For severe deficiency: 50,000 IU once weekly for 8-12 weeks, followed by maintenance dose. For maintenance: 800-2000 IU daily or equivalent weekly/monthly dose.
Elderly
Same as adults, but monitoring is crucial due to potential for renal impairment.
Renal_impairment
Use with caution. Dosage adjustment may be required. Active forms of vitamin D may be preferred in severe renal failure.
How to Take
Oral. Take the capsule with a meal or a fatty snack to enhance absorption. Do not chew or crush the capsule.
Mechanism of Action
Cholecalciferol is converted in the liver to 25-hydroxycholecalciferol (calcidiol) and then in the kidneys to 1,25-dihydroxycholecalciferol (calcitriol), the biologically active form. Calcitriol acts as a hormone to regulate calcium and phosphate metabolism, promoting absorption from the gut, reabsorption in the kidneys, and bone mineralization.
Pharmacokinetics
Onset
Weeks to months for significant clinical effect, depending on baseline levels and dosage.
Excretion
Mainly via bile and feces; small amounts excreted in urine.
Half life
Approximately 19-25 hours for cholecalciferol; calcidiol has a half-life of 2-3 weeks; calcitriol has a half-life of 3-6 hours.
Absorption
Readily absorbed from the small intestine, primarily in the jejunum, in the presence of bile salts.
Metabolism
Primarily hepatic (liver) hydroxylation to 25-hydroxycholecalciferol (calcidiol), then renal (kidney) hydroxylation to 1,25-dihydroxycholecalciferol (calcitriol).
Side Effects
Contraindications
- •Hypersensitivity to Cholecalciferol or any components
- •Hypercalcemia (high calcium levels)
- •Hypervitaminosis D (vitamin D toxicity)
- •Severe renal impairment with hyperphosphatemia
Drug Interactions
Corticosteroids
May reduce Vitamin D activity.
Thiazide diuretics
May increase risk of hypercalcemia.
Cardiac glycosides (e.g., Digoxin)
Hypercalcemia from Vitamin D can increase the risk of digoxin toxicity.
Anticonvulsants (e.g., Phenytoin, Barbiturates)
May reduce the effect of Vitamin D by accelerating its metabolism.
Cholestyramine, Colestipol, Orlistat, Mineral oil
May impair absorption of Vitamin D.
Storage
Store below 30°C in a dry place, away from direct sunlight. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia, hypercalciuria, nausea, vomiting, loss of appetite, excessive thirst, frequent urination, muscle weakness, bone pain, and kidney stones. Treatment involves discontinuing the supplement, hydrating the patient, and managing hypercalcemia under medical supervision.
Pregnancy & Lactation
Pregnancy Category C. Vitamin D is essential during pregnancy and lactation. However, high doses should be avoided as they may be harmful to the fetus. Consult a doctor for appropriate dosage.
