Description
Overview of the medicine
D-Balance 200,000 IU is a high-dose Cholecalciferol (Vitamin D3) preparation used for the treatment and prevention of vitamin D deficiency.
Uses & Indications
Dosage
Adults
For severe vitamin D deficiency: 200,000 IU as a single dose, orally or intramuscularly. May be repeated after 1-3 months based on serum 25(OH)D levels and clinical response.
Elderly
Same as adult dosage, close monitoring of calcium levels is recommended due to potential increased risk of hypercalcemia.
Renal_impairment
Use with caution. Monitor serum calcium and phosphate levels. Dosage adjustments may be necessary based on individual patient response and severity of renal impairment.
How to Take
The solution can be taken orally directly from the vial, or it can be administered intramuscularly by a healthcare professional. Oral administration should preferably be with a fatty meal to enhance absorption.
Mechanism of Action
Cholecalciferol is metabolised in the liver to 25-hydroxycholecalciferol and then in the kidneys to 1,25-dihydroxycholecalciferol (calcitriol), the active form of vitamin D. Calcitriol regulates calcium and phosphate homeostasis, promoting their absorption from the gut and deposition in bones.
Pharmacokinetics
Onset
Clinical effects may be observed within days to weeks of administration.
Excretion
Primarily excreted via bile into the feces, with a small amount excreted in urine.
Half life
Parent compound (cholecalciferol): 24-48 hours. Active metabolite (calcitriol): 3-6 hours. Circulating 25-hydroxyvitamin D has a half-life of 2-3 weeks.
Absorption
Well absorbed from the small intestine, absorption is enhanced by fat.
Metabolism
Hydroxylated in the liver to 25-hydroxycholecalciferol (calcifediol) and then in the kidneys to 1,25-dihydroxycholecalciferol (calcitriol).
Side Effects
Contraindications
- •Hypercalcemia (high blood calcium levels)
- •Vitamin D toxicity
- •Hypersensitivity to Cholecalciferol or any components of the preparation
- •Severe renal impairment with hypercalciuria (unless hypercalcemia is controlled)
Drug Interactions
Corticosteroids
May reduce the effects of vitamin D.
Thiazide diuretics
May reduce urinary calcium excretion and increase the risk of hypercalcemia.
Cardiac glycosides (e.g., digoxin)
Hypercalcemia can potentiate the effects of cardiac glycosides, increasing the risk of arrhythmias.
Anticonvulsants (e.g., phenytoin, barbiturates)
May increase the metabolism of vitamin D, potentially reducing its effectiveness.
Ion exchange resins (e.g., cholestyramine), paraffin oil
May impair absorption of vitamin D.
Storage
Store below 30°C in a dry place. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia (nausea, vomiting, loss of appetite, constipation, weakness, fatigue, polyuria, polydipsia). Severe cases can lead to renal impairment and cardiac arrhythmias. Treatment involves discontinuing vitamin D, hydration, low calcium diet, and potentially corticosteroids or bisphosphonates.
Pregnancy & Lactation
During pregnancy and lactation, vitamin D supplementation should be carefully monitored and within recommended daily allowances. High doses like 200,000 IU are generally not recommended unless specifically prescribed by a physician due to severe deficiency, with careful monitoring of maternal and fetal calcium levels.
