Uses & Indications
Dosage
Adults
For Vitamin D deficiency, 2000-4000 IU daily or as directed by physician. For maintenance, 600-2000 IU daily.
Elderly
Similar to adult dosage, but may require higher doses due to reduced skin synthesis and absorption.
Renal_impairment
Use with caution; close monitoring of calcium and phosphate levels. Dosage may need adjustment based on severity and physician's advice.
How to Take
Oral administration. Tablets can be taken with or without food, but absorption is enhanced when taken with a meal containing fat.
Mechanism of Action
Cholecalciferol (Vitamin D3) is biologically inactive and must be hydroxylated in the liver to 25-hydroxycholecalciferol (calcidiol) and then in the kidney to 1,25-dihydroxycholecalciferol (calcitriol), the active form. Calcitriol acts by binding to the Vitamin D receptor (VDR), regulating gene expression to promote calcium and phosphate absorption in the intestine, reabsorption in the kidney, and bone mineralization.

Effects on calcium and phosphate levels typically seen within 1-2 days, but full therapeutic effect may take weeks.
Excretion
Mainly excreted in the bile and feces; small amounts in urine.
Half life
Plasma half-life of 25(OH)D3 is approximately 15-30 days; active 1,25(OH)2D3 has a half-life of 4-6 hours.
Absorption
Readily absorbed from the small intestine in the presence of bile salts; absorption may be decreased in malabsorption syndromes.
Metabolism
Primarily metabolized in the liver to 25-hydroxycholecalciferol (calcidiol), then in the kidneys to the active form, 1,25-dihydroxycholecalciferol (calcitriol).

Drug Interactions
Cardiac glycosides

Orlistat, Cholestyramine, Mineral oil
May decrease absorption of Vitamin D.
Storage
Store below 30°C in a cool, dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcaemia, nausea, vomiting, weakness, thirst, polyuria, anorexia, headache, and confusion. Management involves discontinuing Vitamin D, low calcium diet, and increasing fluid intake. Severe cases may require medical intervention.
Pregnancy & Lactation
Safe for use during pregnancy and lactation within recommended daily allowances. High doses should be avoided unless prescribed by a doctor due to potential risks of hypercalcaemia in the fetus/infant.
