Description
Overview of the medicine
Cholecalciferol (Vitamin D3) is a fat-soluble vitamin essential for calcium absorption, bone health, and immune system function.
Uses & Indications
Dosage
Adults
For severe deficiency, 20,000 IU once weekly for 7 weeks, followed by a maintenance dose as directed by a physician. Typical maintenance is 800-2000 IU daily.
Elderly
Similar to adult dosage, but caution with renal impairment and hypercalcemia risk. Often requires careful monitoring.
Renal_impairment
Use with caution. Dosage adjustments may be needed under strict medical supervision due to altered metabolism. Active form (calcitriol) may be preferred.
How to Take
Take orally with food, preferably with the largest meal of the day, to enhance absorption.
Mechanism of Action
Cholecalciferol is converted to calcitriol (1,25-dihydroxyvitamin D3) in the body, which acts as a hormone to regulate calcium and phosphate homeostasis.
Pharmacokinetics
Onset
Days to weeks for clinical effects, depending on baseline deficiency and dosage regimen.
Excretion
Mainly excreted in bile and feces; minimal renal excretion.
Half life
Cholecalciferol: ~24 hours; its main metabolite, 25-hydroxyvitamin D (calcidiol), has a half-life of 2-3 weeks.
Absorption
Readily absorbed from the small intestine in the presence of bile salts, primarily from the duodenum and jejunum.
Metabolism
Hydroxylated in the liver to 25-hydroxyvitamin D (calcidiol) and then in the kidneys to 1,25-dihydroxyvitamin D (calcitriol), the active form.
Side Effects
Contraindications
- •Hypersensitivity to Vitamin D or any component of the formulation
- •Hypercalcemia
- •Hypervitaminosis D
- •Nephrolithiasis (kidney stones) due to hypercalcemia
Drug Interactions
Corticosteroids
May reduce Vitamin D effects and calcium absorption.
Thiazide diuretics
May increase risk of hypercalcemia by reducing urinary calcium excretion.
Phenytoin, Barbiturates, Carbamazepine
May increase Vitamin D metabolism, reducing its effects, necessitating higher doses.
Cholestyramine, Colestipol, Orlistat, Mineral oil
May impair absorption of fat-soluble vitamins, including Vitamin D.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms include hypercalcemia (nausea, vomiting, constipation, polyuria, polydipsia, weakness, confusion, fatigue). Treatment involves discontinuing the drug, low calcium diet, hydration, and potentially corticosteroids or loop diuretics in severe cases.
Pregnancy & Lactation
Generally safe within recommended daily allowances. High doses should be used with caution and only if clearly needed under strict medical supervision.
