Description
Overview of the medicine
Dydrogesterone is a synthetic progestogen used to treat a variety of gynaecological conditions caused by a lack of natural progesterone. These include threatened and habitual miscarriage, endometriosis, dysfunctional uterine bleeding, and as part of hormone replacement therapy.
Uses & Indications
Dosage
Adults
Threatened miscarriage: 40 mg initially, then 10 mg every 8 hours until symptoms subside. Habitual miscarriage: 10 mg twice daily until the 20th week of pregnancy. Endometriosis: 10 mg 2-3 times daily from day 5 to day 25 of the cycle, or continuously. HRT: 10-20 mg daily for 14 days of a 28-day cycle, or continuously with estrogen.
Elderly
No specific dose adjustment generally required, but caution with hepatic/renal impairment.

How to Take
To be taken orally, with or without food. Follow the physician's instructions regarding timing, especially in relation to the menstrual cycle for specific indications.
Mechanism of Action
Dydrogesterone acts directly on the uterus, mimicking the effects of natural progesterone. It binds to progesterone receptors, promoting the development of a secretory endometrium in estrogen-primed uteri. It also inhibits estrogen-induced endometrial proliferation.

Drug Interactions
Ritonavir, Nelfinavir
May increase dydrogesterone levels.
Enzyme Inducers (e.g., Phenobarbital, Phenytoin, Rifampicin, Carbamazepine, St. John's Wort)
May decrease dydrogesterone levels and efficacy.
Storage

Overdose
Overdose symptoms are generally mild, such as nausea, vomiting, dizziness, and drowsiness. Treatment is symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Pregnancy: Dydrogesterone is indicated for use in pregnancy in cases of threatened or habitual miscarriage under medical supervision. Lactation: Dydrogesterone is excreted in breast milk and is generally not recommended during breastfeeding.
