Overview of the medicine
Metronidazole is an antimicrobial medication used to treat a variety of bacterial and parasitic infections. It belongs to the nitroimidazole class of antibiotics.
Uses & Indications
Dosage
Adults
Amoebiasis: 500-750 mg orally three times daily for 5-10 days. Trichomoniasis: 2 g orally as a single dose or 250 mg three times daily for 7 days. Anaerobic infections: 500 mg orally/IV every 8 hours.
Elderly
Dosage adjustments may be necessary due to decreased hepatic function. Monitor closely.
Renal_impairment
No routine dosage adjustment for mild to moderate impairment. For severe, reduce dosage.
How to Take
Oral tablets should be taken with food to minimize gastric upset. Oral suspension should be shaken well before use. IV infusion should be administered slowly.
Mechanism of Action
Metronidazole enters the cell of susceptible microorganisms and is reduced to a highly reactive nitro radical. This radical damages DNA and inhibits nucleic acid synthesis, leading to cell death.

Onset
Varies by infection type and route; usually within hours for symptomatic relief.
Excretion
Mainly renal (60-80% as metabolites, 20% unchanged); small amount in feces.
Half life
Approximately 8 hours (range 6-12 hours).

Side Effects
Contraindications
- •Hypersensitivity to metronidazole or other nitroimidazole derivatives
- •First trimester of pregnancy (relative contraindication, especially for trichomoniasis)
- •Concurrent use of disulfiram
- •Active central nervous system disease
Drug Interactions
Alcohol
Disulfiram-like reaction (nausea, vomiting, flushing, tachycardia).
Lithium
Increases plasma lithium levels, leading to toxicity.
Warfarin
Potentiates anticoagulant effect, increasing bleeding risk.
Cimetidine
May prolong half-life of metronidazole.
Disulfiram
Psychotic reactions (contraindicated).
Storage
Store below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, ataxia, and mild disorientation. Treatment is symptomatic and supportive; no specific antidote.
Pregnancy & Lactation
