Description
Overview of the medicine
Osteo-D 2000 IU Chewable Tablet contains Cholecalciferol (Vitamin D3) and is used to treat and prevent Vitamin D deficiency and associated conditions like osteoporosis and osteomalacia. It supports calcium absorption and bone health.
Uses & Indications
Dosage
Adults
For Vitamin D deficiency, typically 2000 IU daily. Doses may vary based on serum 25(OH)D levels and clinical condition, as advised by a physician. For maintenance, 800-2000 IU daily.
Elderly
Similar to adult dosage, but may require closer monitoring due to potential underlying conditions and drug interactions. Daily intake of 800-2000 IU is often recommended.
Renal_impairment
Use with caution. High doses may lead to hypercalcemia. Monitoring of calcium and phosphate levels is crucial. May require activated forms of Vitamin D if renal impairment is severe.
How to Take
Chew the tablet completely before swallowing, preferably with a meal to enhance absorption. Do not swallow whole.
Mechanism of Action
Cholecalciferol (Vitamin D3) is a precursor that is metabolized in the liver and kidneys to calcitriol, the active form of Vitamin D. Calcitriol regulates calcium and phosphate homeostasis by promoting their absorption in the intestine, enhancing bone mineralization, and affecting renal tubular reabsorption.
Pharmacokinetics
Onset
Weeks to months for significant changes in serum 25(OH)D levels.
Excretion
Mainly excreted in bile and feces, with a small amount in urine.
Half life
Initial half-life of Cholecalciferol is 24 hours, but the active metabolite 25(OH)D3 has a half-life of approximately 2-3 weeks, and 1,25(OH)2D3 (calcitriol) has a half-life of 4-6 hours.
Absorption
Readily absorbed from the gastrointestinal tract, especially in the presence of bile salts and dietary fats.
Metabolism
Hydroxylated in the liver to 25-hydroxycholecalciferol (calcidiol) and then in the kidneys to 1,25-dihydroxycholecalciferol (calcitriol), the active form.
Side Effects
Contraindications
- Hypersensitivity to Cholecalciferol or any components of the tablet.
- Hypercalcemia (high levels of calcium in the blood).
- Hypervitaminosis D (excessively high levels of Vitamin D).
- Severe renal impairment (unless managed with activated Vitamin D forms).
Drug Interactions
Corticosteroids
May reduce the effects of Vitamin D.
Thiazide diuretics
May increase the risk of hypercalcemia.
Cholestyramine, mineral oil, orlistat
May reduce absorption of Vitamin D.
Anticonvulsants (e.g., phenytoin, barbiturates)
May increase Vitamin D metabolism and reduce its effectiveness.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia, nausea, vomiting, constipation, polyuria, polydipsia, and muscle weakness. Treatment involves discontinuing Vitamin D, hydrating the patient, and managing hypercalcemia.
Pregnancy & Lactation
Vitamin D is essential during pregnancy and lactation. However, doses should be carefully monitored to avoid hypervitaminosis D, which can be harmful to the fetus or infant. Consult a doctor for appropriate dosage.
