Description
Overview of the medicine
Osteo-D 40000 IU Capsule contains Cholecalciferol (Vitamin D3), a fat-soluble vitamin essential for calcium and phosphate absorption, bone health, and immune system function. It is primarily used to treat and prevent Vitamin D deficiency and associated conditions.
Uses & Indications
Dosage
Adults
For severe deficiency: 40000 IU once weekly for 6-10 weeks, then maintenance dose. Specific dosage should be determined by a healthcare professional based on Vitamin D levels.
Elderly
Similar to adult dosage, but caution should be exercised. Renal function should be monitored.
Renal_impairment
Use with caution. Dosage adjustment may be required based on serum calcium and phosphate levels. Not recommended in severe renal failure.
How to Take
Swallow the capsule whole with water. It can be taken with or without food, but absorption may be better with a meal containing fat.
Mechanism of Action
Cholecalciferol (Vitamin D3) is converted in the liver to 25-hydroxycholecalciferol (calcidiol) and then in the kidneys to 1,25-dihydroxycholecalciferol (calcitriol), the biologically active form. Calcitriol acts on intestinal cells to increase calcium and phosphate absorption, on bones to regulate calcium metabolism, and on kidneys to reduce calcium excretion, thus maintaining serum calcium and phosphate levels.
Pharmacokinetics
Onset
Therapeutic effects may take several days to weeks to manifest, as body stores need to be replenished and active metabolite formed.
Excretion
Primarily excreted in bile and feces. A small amount is excreted in urine.
Half life
The half-life of 25-hydroxycholecalciferol is about 15-30 days; the half-life of 1,25-dihydroxycholecalciferol is 3-6 hours.
Absorption
Readily absorbed from the small intestine, primarily in the presence of bile salts and dietary fat. Peak plasma levels reached within 4-5 hours.
Metabolism
Metabolized in the liver to 25-hydroxycholecalciferol (calcidiol) and then in the kidneys to 1,25-dihydroxycholecalciferol (calcitriol).
Side Effects
Contraindications
- Hypersensitivity to Cholecalciferol or any components of the capsule
- Hypercalcemia
- Hypervitaminosis D
- Nephrolithiasis
- Severe renal impairment
Drug Interactions
Corticosteroids
May reduce the effects of Vitamin D.
Thiazide diuretics
May increase the risk of hypercalcemia.
Phenytoin/Barbiturates
May increase Vitamin D metabolism, reducing its effect.
Orlistat and Cholestyramine
May reduce absorption of Vitamin D.
Cardiac glycosides (e.g., Digoxin)
Risk of arrhythmia is increased with hypercalcemia.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia (nausea, vomiting, weakness, headache, thirst, increased urination, constipation), hypercalciuria, and renal impairment. Management involves discontinuing Vitamin D, reducing calcium intake, and hydration. Severe cases may require hospitalization and intravenous fluids.
Pregnancy & Lactation
Vitamin D is essential during pregnancy and lactation. High doses should only be taken under medical supervision to avoid hypercalcemia in the mother, which could affect the fetus or infant. Consult a doctor before use.

Clinical Trials
Numerous clinical trials support the efficacy of Cholecalciferol in treating and preventing Vitamin D deficiency and related bone disorders. Ongoing research explores its role in immune function, cardiovascular health, and certain cancers.
Lab Monitoring
- Serum 25-hydroxyvitamin D levels
- Serum calcium levels
- Serum phosphate levels
- Renal function tests (creatinine, BUN)
Doctor Notes
- Always check serum 25(OH)D, calcium, and phosphate levels before initiating and during high-dose Vitamin D therapy.
- Educate patients on symptoms of hypercalcemia.
- Consider co-administration with calcium if dietary intake is insufficient and no hypercalcemia risk.
Patient Guidelines
- Take exactly as prescribed by your doctor.
- Do not exceed the recommended dose.
- Inform your doctor about all other medications you are taking.
- Report any unusual symptoms, especially signs of hypercalcemia (e.g., nausea, vomiting, excessive thirst, frequent urination).
- Maintain a balanced diet and regular sun exposure as advised by your doctor.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Cholecalciferol is generally not expected to impair the ability to drive or operate machinery. However, if side effects like weakness or dizziness occur, avoid such activities.
Lifestyle Advice
- Engage in regular weight-bearing exercise to support bone health.
- Ensure adequate dietary calcium intake through dairy products, leafy greens, or supplements if necessary.
- Moderate sun exposure can help natural Vitamin D production, but be mindful of skin protection.
- Avoid smoking and excessive alcohol consumption, as these can negatively affect bone health.
