Description
Overview of the medicine
Renova (Tretinoin) cream is a topical retinoid primarily used to reduce fine wrinkles, mottled hyperpigmentation, and tactile roughness of facial skin due to photoaging. It is also used for the treatment of acne vulgaris.
Uses & Indications
Dosage
Adults
Apply a pea-sized amount to the entire affected area of the face once daily in the evening, after washing and drying the skin. Gently massage into the skin.
Elderly
Same as adults, with careful monitoring for skin irritation.
Renal_impairment
No specific dosage adjustment needed for topical use.
How to Take
Apply to clean, dry skin. Wash your face with a mild cleanser and pat dry before application. Use once daily, typically at bedtime. Avoid applying near eyes, mouth, and nostrils. Allow 20-30 minutes after washing your face before applying to ensure skin is completely dry.
Mechanism of Action
Tretinoin works by binding to retinoid receptors (RARs and RXRs) in skin cells, influencing gene expression that normalizes cell differentiation, proliferation, and reduces inflammation. It promotes increased cell turnover, reduces the cohesion of follicular epithelial cells, and enhances the shedding of old cells.
Pharmacokinetics
Onset
Effects on acne can be seen within 2-3 weeks, but significant improvement for photoaging may take 3-6 months.
Excretion
If absorbed, excreted in urine and feces.
Half life
Systemic half-life is approximately 0.75 hours (if absorbed), but topical effects are sustained.
Absorption
Minimal systemic absorption after topical application. Approximately 1.6% to 5.4% is absorbed through the skin.
Metabolism
If absorbed, metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- Pregnancy (Category C, but often avoided due to potential risks)
- Hypersensitivity to tretinoin or any excipients
- Severe sunburn or acutely inflamed skin
Drug Interactions
Photosensitizing agents
Increased risk of sunburn with other medications that increase sensitivity to sunlight (e.g., thiazides, tetracyclines, fluoroquinolones, phenothiazines).
Other topical irritating medications
Concomitant use with other potentially irritating topical products (e.g., medicated soaps, astringents, alcohol-containing products, sulfur, resorcinol, salicylic acid, benzoyl peroxide) should be approached with caution as it may lead to increased irritation. If used, apply at different times of the day.
Storage
Store at room temperature (below 25°C), away from direct light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Excessive topical application of tretinoin may result in marked redness, peeling, or discomfort. Systemic overdose is unlikely due to minimal absorption. Treatment should be symptomatic.
Pregnancy & Lactation
Tretinoin is generally contraindicated in pregnancy (Category C) due to potential teratogenic effects, especially with systemic exposure. Although topical absorption is minimal, it is advised to avoid use. For lactation, it is unknown if tretinoin is excreted in human milk; use with caution and only if clearly needed after consulting a doctor.
