Description
Overview of the medicine
Sefur 125 mg Tablet contains Cefuroxime Axetil, a second-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections including respiratory tract infections, ear infections, skin and soft tissue infections, and urinary tract infections.
Uses & Indications
Dosage
Adults
For mild to moderate infections: 125-250 mg twice daily. For severe infections (e.g., bronchitis, pneumonia): 250-500 mg twice daily (using higher strength tablets). For uncomplicated urinary tract infections: 125 mg twice daily.
Elderly
Same as adults, but dose adjustment may be necessary in case of renal impairment.
Renal_impairment
Dose adjustment is required based on creatinine clearance. For CrCl 10-29 mL/min, 125-250 mg once daily. For CrCl <10 mL/min, 125-250 mg every 48 hours.
How to Take
Take orally, preferably with food to enhance absorption and reduce gastrointestinal discomfort. Do not crush or chew the tablet; swallow it whole.
Mechanism of Action
Cefuroxime Axetil is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), thus preventing the cross-linking of peptidoglycan chains, which is essential for bacterial cell wall integrity and strength, leading to cell lysis and death.

Oral absorption peaks approximately 2-3 hours after administration with food.
Excretion
Excreted unchanged in the urine, primarily by glomerular filtration and tubular secretion.
Half life
Approximately 1 to 1.5 hours in adults with normal renal function.
Absorption
Cefuroxime axetil is well absorbed from the gastrointestinal tract and rapidly hydrolyzed by esterases to cefuroxime. Absorption is enhanced when administered with food.
Metabolism
Cefuroxime axetil is rapidly hydrolyzed in the intestinal mucosa and blood to the active moiety, cefuroxime. It is not significantly metabolized in the liver.

Contraindications
- Known hypersensitivity to cefuroxime, other cephalosporins, or any component of the formulation
- History of severe hypersensitivity reaction to any other beta-lactam antibiotic (e.g., penicillins)
Drug Interactions
Probenecid
Increases cefuroxime plasma concentrations and prolongs its duration of action by reducing renal tubular secretion.
Oral Contraceptives
May reduce the efficacy of oral contraceptives by altering gut flora; additional barrier contraception is advised.
Antacids/H2 blockers/PPIs
May decrease the bioavailability of cefuroxime axetil, leading to reduced absorption. It is recommended to administer cefuroxime axetil at least 2 hours before or 4 hours after antacids/H2 blockers.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include cerebral irritation leading to convulsions. Management is supportive and symptomatic; hemodialysis may be beneficial in cases of severe overdose.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of impaired fertility or harm to the fetus. Cefuroxime is excreted in breast milk in small quantities; use with caution during lactation. Consult a doctor before use.
