Overview of the medicine
Cefuroxime axetil is a second-generation cephalosporin antibiotic used to treat a wide range of bacterial infections including respiratory tract, urinary tract, skin, and soft tissue infections.
Uses & Indications
Dosage
Adults
250 mg to 500 mg twice daily, depending on the severity and type of infection. For most infections, 250 mg twice daily. For more severe infections or lower respiratory tract infections, 500 mg twice daily. For uncomplicated urinary tract infections, 125 mg to 250 mg twice daily.
Elderly
Dosage adjustment is not typically required in elderly patients with normal renal function.
Renal_impairment
Dosage adjustment is necessary for severe renal impairment (CrCl < 30 mL/min). A reduced frequency or dose may be required.
How to Take
Administer orally. For optimal absorption, it is recommended to take Cefuroxime axetil tablets with food.
Mechanism of Action
Cefuroxime axetil exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), disrupting peptidoglycan synthesis, leading to cell lysis and bacterial death.

Onset
Within hours for therapeutic effect.
Excretion
Excreted unchanged by glomerular filtration and tubular secretion in the urine.
Half life
Approximately 1 to 1.5 hours in adults with normal renal function.
Absorption
Rapidly absorbed from the gastrointestinal tract and rapidly hydrolyzed by esterases in the intestinal mucosa and blood to cefuroxime. Peak plasma concentrations are achieved 2-3 hours after oral administration.
Metabolism
Not significantly metabolized in the liver; it is hydrolyzed to the active compound cefuroxime.
Side Effects
Contraindications
- Hypersensitivity to cefuroxime or other cephalosporins.
- History of severe hypersensitivity (e.g., anaphylaxis) to any type of beta-lactam antibacterial agents (e.g., penicillins, monobactams, carbapenems).
Drug Interactions
Probenecid
May increase cefuroxime plasma levels and prolong its duration of action.
Oral anticoagulants
May rarely enhance anticoagulant effect.
Oral contraceptives
May reduce efficacy.
Antacids/H2 blockers/Proton pump inhibitors
May reduce bioavailability by decreasing gastric acidity.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include cerebral irritation, convulsions. Treatment is supportive; hemodialysis may be beneficial.

Pregnancy Category B. Use only if clearly needed. Excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but monitor infant for potential side effects (e.g., diarrhea).
