Overview of the medicine
Sinaceph 1 gm injection contains Cefotaxime, a broad-spectrum cephalosporin antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
Typical dose is 1 gm IV/IM every 8-12 hours. In severe infections, doses up to 2 gm IV every 6-8 hours (maximum 12 gm/day) may be given.
Elderly
Dosage adjustments may be necessary based on renal function and overall health status.
Renal_impairment
In severe renal impairment (creatinine clearance < 20 ml/min), the maintenance dose should be halved (e.g., 0.5-1 gm every 12-24 hours).
How to Take
Sinaceph 1 gm injection can be administered either by slow intravenous (IV) injection over 3-5 minutes, IV infusion over 20-60 minutes, or deep intramuscular (IM) injection.
Mechanism of Action
Cefotaxime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid, within minutes of intravenous administration.
Excretion
Mainly excreted by the kidneys, with approximately 40-60% as unchanged drug and 20% as desacetylcefotaxime within 24 hours.
Half life
Approximately 1 to 1.5 hours for Cefotaxime, and 1.5 to 2 hours for its active metabolite, desacetylcefotaxime.
Absorption
Rapidly absorbed after intramuscular (IM) or intravenous (IV) administration. Peak plasma concentrations are reached within 30 minutes after IV and 1 hour after IM administration.
Metabolism
Primarily metabolized in the liver to an active metabolite, desacetylcefotaxime, and two inactive metabolites.
Side Effects
Contraindications
- •Known hypersensitivity to cefotaxime, other cephalosporins, or any component of the formulation.
- •History of severe hypersensitivity reaction to any other beta-lactam antibiotic (e.g., penicillins, carbapenems).
Drug Interactions
Probenecid
Increases and prolongs the plasma concentration of cefotaxime by reducing its renal tubular secretion.
Loop Diuretics
May increase the risk of ototoxicity and nephrotoxicity when given concurrently with aminoglycosides and other nephrotoxic drugs.
Aminoglycosides
Concurrent use may increase the risk of nephrotoxicity. Renal function should be monitored.
Storage
Store below 30°C in a dry place, protected from light. Keep out of reach of children. Reconstituted solution should be used immediately or within 24 hours if refrigerated.
Overdose
Symptoms of overdose may include encephalopathy (especially with high doses), convulsions, or neuromuscular excitability. Treatment is symptomatic and supportive. Hemodialysis may be helpful in severe cases, especially in patients with impaired renal function.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Cefotaxime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.
