Sinaceph 500 mg capsule contains Cephalexin, a first-generation cephalosporin antibiotic used to treat various bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
The usual adult dose is 250 mg to 1 g every 6 hours, depending on the severity of the infection. For most infections, 500 mg every 8 hours is effective.
Elderly
No specific dose adjustment for elderly patients with normal renal function. Dosage should be adjusted based on renal function.
Renal_impairment
For patients with creatinine clearance < 10 mL/min, the dose should not exceed 250 mg every 12 hours. For 10-40 mL/min, 250 mg every 8-12 hours.
How to Take
Sinaceph capsule should be taken orally with or without food. For best absorption and to reduce gastrointestinal upset, it can be taken with food. Complete the full course of treatment as prescribed by the doctor.
Mechanism of Action
Cephalexin is a bactericidal agent that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). This leads to the lysis and death of susceptible bacteria.
Pharmacokinetics
Onset
Rapid, typically within 1 hour.
Excretion
Mainly by the kidneys (80-90% as unchanged drug) within 8 hours via glomerular filtration and tubular secretion.
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract (about 90%). Peak plasma concentrations are reached within 1 hour after administration.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- •Known hypersensitivity to cephalexin or other cephalosporin antibiotics.
- •History of severe hypersensitivity reaction to any beta-lactam antibiotic (e.g., penicillin).
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin. Monitor INR closely.
Metformin
Cephalexin may increase metformin concentrations, potentially increasing the risk of lactic acidosis.
Probenecid
May decrease the renal clearance of cephalexin, leading to increased and prolonged cephalexin concentrations.
Live bacterial vaccines (e.g., typhoid vaccine)
Antibiotics may reduce the therapeutic effect of these vaccines.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, diarrhea, and hematuria. Treatment is primarily supportive. Gastric lavage or activated charcoal may be considered. Hemodialysis or peritoneal dialysis may be helpful in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Cephalexin is excreted in breast milk in small amounts; use with caution in breastfeeding mothers.
