Cefalexin is a first-generation cephalosporin antibiotic used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
Not typically used in this strength for adults. For children:
Elderly
No specific dose adjustment for elderly with normal renal function; follow pediatric guidelines.
Renal_impairment
Dosage adjustment is necessary for patients with impaired renal function. Consult physician.
How to Take
Administer orally. Shake the bottle well before each use. Can be taken with or without food. Use a calibrated measuring spoon or cup for accurate dosing.
Mechanism of Action
Cefalexin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and bacterial death.

Mainly excreted unchanged by the kidneys via glomerular filtration and tubular secretion.

Side Effects
Contraindications
- •Known hypersensitivity to cefalexin or any other cephalosporin antibiotics.
- •History of severe hypersensitivity reactions to penicillin (e.g., anaphylaxis).
Drug Interactions
Metformin
Cefalexin may increase plasma concentrations of metformin. Monitor blood glucose levels.
Probenecid
May increase and prolong serum cefalexin concentrations by decreasing renal tubular secretion.
Oral Contraceptives
May reduce the efficacy of oral contraceptives in rare cases. Additional contraception may be advised.
Anticoagulants (e.g., Warfarin)
Increased INR/PT has been reported in patients receiving cefalexin and anticoagulants concurrently; monitor coagulation parameters.
Storage
Store the unmixed powder at room temperature, below 30°C, away from moisture and direct light. After reconstitution, store the suspension in a refrigerator (2-8°C) and discard after 14 days.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, diarrhea, and hematuria. Management is primarily supportive; activated charcoal may be used to reduce absorption if recent. Hemodialysis may be useful in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but adequate and well-controlled studies in pregnant women are lacking. Use during pregnancy only if clearly needed. Cefalexin is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.
