Adults

For most infections, 750 mg to 1.5 g IV/IM every 8 hours. For Sinaceph-500 mg, a typical dose might be 500 mg IV/IM every 8 hours for less severe infections or as part of a tailored regimen.

Elderly

Dosage adjustment may be necessary based on renal function. Generally, the same as adults unless renal impairment is present.

Renal_impairment

Dose reduction is required in patients with significantly impaired renal function based on creatinine clearance. Consult specific guidelines.

Onset

Rapid, typically within minutes for IV administration and 15-60 minutes for IM administration.

Excretion

Primarily excreted unchanged by the kidneys via glomerular filtration and tubular secretion. Approximately 89% of the administered dose is excreted within 24 hours.

Half life

Approximately 1 to 1.5 hours in adults with normal renal function.

Absorption

Rapidly absorbed after intramuscular injection, peak plasma concentrations reached within 30-60 minutes. Intravenous administration achieves immediate peak concentrations.

Metabolism

Cefuroxime is not significantly metabolized in the body.

Probenecid

Concomitant administration with probenecid prolongs the renal tubular secretion of cefuroxime, resulting in higher and more prolonged plasma concentrations.

Aminoglycosides

Concurrent use of high doses of cephalosporins with aminoglycosides or potent diuretics (e.g., furosemide) may increase the risk of nephrotoxicity.

Oral Anticoagulants

Cefuroxime may potentiate the anticoagulant effect of oral anticoagulants (e.g., warfarin), requiring close monitoring of INR.
