Description
Overview of the medicine
Cefuroxime is a broad-spectrum cephalosporin antibiotic used to treat a wide range of bacterial infections. Sinaceph-500 mg injection is administered intravenously or intramuscularly.
Uses & Indications
Dosage
Adults
For most infections, 750 mg to 1.5 g IV/IM every 8 hours. For Sinaceph-500 mg, a typical dose might be 500 mg IV/IM every 8 hours for less severe infections or as part of a tailored regimen.
Elderly
Dosage adjustment may be necessary based on renal function. Generally, the same as adults unless renal impairment is present.
Renal_impairment
Dose reduction is required in patients with significantly impaired renal function based on creatinine clearance. Consult specific guidelines.
How to Take
For intravenous administration, inject slowly over 3-5 minutes or infuse over 20-30 minutes. For intramuscular administration, inject deeply into a large muscle mass (e.g., gluteus or thigh). Reconstitute the powder with appropriate diluent (e.g., sterile water for injection).
Mechanism of Action
Cefuroxime acts by inhibiting the synthesis of the bacterial cell wall, leading to bacterial lysis and death. It has bactericidal activity against many Gram-positive and Gram-negative bacteria.
Pharmacokinetics
Onset
Rapid, typically within minutes for IV administration and 15-60 minutes for IM administration.
Excretion
Primarily excreted unchanged by the kidneys via glomerular filtration and tubular secretion. Approximately 89% of the administered dose is excreted within 24 hours.
Half life
Approximately 1 to 1.5 hours in adults with normal renal function.
Absorption
Rapidly absorbed after intramuscular injection, peak plasma concentrations reached within 30-60 minutes. Intravenous administration achieves immediate peak concentrations.
Metabolism
Cefuroxime is not significantly metabolized in the body.
Side Effects
Contraindications
- •Hypersensitivity to cefuroxime or any other cephalosporin antibiotics.
- •History of severe hypersensitivity reactions (e.g., anaphylaxis) to any other type of beta-lactam antibacterial agent (e.g., penicillins, carbapenems).
Drug Interactions
Probenecid
Concomitant administration with probenecid prolongs the renal tubular secretion of cefuroxime, resulting in higher and more prolonged plasma concentrations.
Aminoglycosides
Concurrent use of high doses of cephalosporins with aminoglycosides or potent diuretics (e.g., furosemide) may increase the risk of nephrotoxicity.
Oral Anticoagulants
Cefuroxime may potentiate the anticoagulant effect of oral anticoagulants (e.g., warfarin), requiring close monitoring of INR.
Storage
Store the dry powder below 30°C, protected from light and moisture. Reconstituted solution should be used immediately or stored as per manufacturer's instructions (usually refrigerated for a limited period).
Overdose
Overdose of cefuroxime can lead to neurological sequelae including convulsions. Management involves supportive care and symptomatic treatment. Hemodialysis can be used to remove cefuroxime from the blood.
Pregnancy & Lactation
Cefuroxime is generally considered safe for use during pregnancy (Category B). It is excreted in low concentrations in breast milk, so caution should be exercised when administering to nursing mothers. Consult a doctor.
