Dosage
Adults
400 mg once daily or 200 mg every 12 hours, typically for 7-14 days depending on infection.
Elderly
No specific dose adjustment for elderly unless renal function is impaired.
Renal_impairment
Dose adjustment required for creatinine clearance < 60 mL/min. For CrCl 21-60 mL/min, 300 mg daily; for CrCl < 20 mL/min, 200 mg daily.
