Piracetam is a nootropic agent that improves cognitive function, memory, and learning by enhancing neuronal activity. It is used to treat conditions associated with cognitive decline and certain neurological disorders.
Uses & Indications
Dosage
Adults
For cognitive impairment: 2.4 g to 4.8 g daily, divided into 2-3 doses. For cortical myoclonus: Initially 7.2 g daily, increasing by 4.8 g every 3-4 days up to a maximum of 24 g daily, divided into 2-3 doses. For vertigo: 2.4 g to 4.8 g daily, divided into 2-3 doses.
Elderly
No specific dose adjustment needed unless renal impairment is present. It is recommended to monitor renal function in elderly patients and adjust dosage accordingly.
Renal_impairment
Dosage adjustment required based on creatinine clearance (CrCl). CrCl 50-79 mL/min: 2/3 of usual dose, 2-3 times daily. CrCl 30-49 mL/min: 1/3 of usual dose, 2 times daily. CrCl < 30 mL/min: Contraindicated or significantly reduced dose.
How to Take
Swallow capsules whole with water, with or without food. For cortical myoclonus, it is crucial to take doses regularly at spaced intervals.
Mechanism of Action
Piracetam enhances neuroplasticity, facilitates interhemispheric transfer of information, and improves microcirculation without causing vasodilation. It may restore normal membrane fluidity and protect neurons from hypoxic damage.

Excretion
Excreted unchanged primarily via the kidneys. Approximately 80-100% of the administered dose is excreted in urine within 24-48 hours.

Approximately 4-5 hours in plasma; 6-8 hours in cerebrospinal fluid.
Absorption
Rapid and complete absorption after oral administration. Bioavailability is nearly 100%. Peak plasma concentrations are reached within approximately 1 hour.
Metabolism
Not metabolized in the human body. Piracetam is chemically stable and does not interact with liver enzymes.

Contraindications
- Hypersensitivity to piracetam or other pyrrolidone derivatives or any excipients of the formulation
- Severe renal impairment (creatinine clearance < 20 mL/min)
- Cerebral hemorrhage
- Huntington's chorea (due to potential for exacerbation)
Drug Interactions
Warfarin
Concomitant use with warfarin may prolong prothrombin time; INR should be closely monitored and warfarin dose adjusted if necessary.
Thyroid hormones
Possible confusion, irritability, and sleep disturbance have been reported when piracetam was used concomitantly with thyroid extracts (T3 + T4).
Anticoagulants/Antiplatelets
May potentiate the effects of these medications; caution advised.

Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of acute overdose, management is symptomatic and supportive. Gastric lavage or induction of emesis may be considered immediately after ingestion. Hemodialysis is an effective method for piracetam removal.
