Description
Overview of the medicine
Azmasol 100 mcg Inhaler contains Salbutamol, a short-acting beta-2 agonist (SABA) used for the rapid relief of symptoms of asthma and chronic obstructive pulmonary disease (COPD) such as wheezing, shortness of breath, and chest tightness.
Uses & Indications
Dosage
Adults
For relief of acute asthma symptoms: 1 or 2 puffs (100 or 200 mcg) as needed. For prevention of exercise-induced bronchospasm: 2 puffs 15-30 minutes before exercise. Do not exceed 8 puffs in 24 hours.
Elderly
Similar to adult dosage, but individual response should be monitored. Start with the lowest effective dose.
Renal_impairment
No specific dosage adjustment is generally required for mild to moderate renal impairment, but caution is advised in severe cases.
How to Take
For oral inhalation only. Shake the inhaler well before each use. Inhale slowly and deeply through the mouthpiece while pressing the canister. Hold your breath for about 10 seconds, then exhale slowly. Rinse your mouth with water after use to reduce the risk of oral thrush.
Mechanism of Action
Salbutamol selectively stimulates beta-2 adrenergic receptors in bronchial smooth muscle, leading to bronchodilation. This relaxes the airways, allowing easier breathing.
Pharmacokinetics
Onset
Bronchodilation typically occurs within 5-15 minutes.
Excretion
Primarily excreted in the urine as unchanged drug (about 60%) and its inactive sulfate metabolite.
Half life
Plasma half-life is 4-6 hours.
Absorption
Rapidly absorbed from the lungs following inhalation, with systemic bioavailability of 10-20% of the inhaled dose.
Metabolism
Partially metabolized in the liver to an inactive sulfate conjugate.
Side Effects
Contraindications
- Hypersensitivity to salbutamol or any of the excipients.
Drug Interactions
Digoxin
Increased risk of digoxin toxicity if hypokalemia occurs.
Diuretics
May increase the risk of hypokalemia with high doses of salbutamol.
MAOIs and TCAs
Concurrent use may potentiate the vascular effects of salbutamol, increasing the risk of adverse cardiovascular events.
Beta-blockers (non-selective)
May reduce the bronchodilatory effect of salbutamol and can precipitate severe bronchospasm.
Storage
Store below 30°C. Protect from direct sunlight and frost. Do not puncture, burn or throw into fire even when empty. Keep out of reach of children.
Overdose
Symptoms of overdose include tachycardia, tremors, headache, and hypokalemia. Management is supportive, and in severe cases, cautious use of a cardioselective beta-blocker may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Salbutamol may be excreted in breast milk; use with caution during lactation.

WHO Essential Medicine
YesClinical Trials
Salbutamol has been extensively studied in numerous clinical trials establishing its efficacy and safety for the treatment of asthma and COPD. These studies support its use as a first-line rescue medication.
Lab Monitoring
- Monitor serum potassium levels, especially in patients receiving high doses or concomitant therapy with diuretics or corticosteroids, due to the risk of hypokalemia.
Doctor Notes
- Emphasize proper inhaler technique to ensure optimal drug delivery and patient education on when and how to use their rescue inhaler.
- Monitor for overuse; increasing reliance on a SABA often indicates poorly controlled asthma, necessitating a review of maintenance therapy.
- Always consider prescribing an inhaled corticosteroid (ICS) for patients with persistent asthma, as SABAs are only for symptomatic relief.
Patient Guidelines
- Ensure proper inhaler technique for effective delivery of the medicine.
- Rinse your mouth with water after each use to prevent oral candidiasis.
- Do not exceed the prescribed dose without consulting your doctor.
- Seek medical attention immediately if your breathing worsens or if you need to use your inhaler more often than usual.
Missed Dose Advice
Azmasol Inhaler is used as needed. If you forget to use it when symptoms appear, take a dose as soon as you remember. Do not take a double dose to make up for a missed one.
Driving Precautions
Azmasol usually does not affect the ability to drive or operate machinery. However, if you experience side effects like tremor or dizziness, avoid these activities.
Lifestyle Advice
- Identify and avoid asthma triggers (e.g., pollen, dust, pet dander).
- Consider smoking cessation if you smoke, as it can worsen respiratory conditions.
