Description
Overview of the medicine
Betadine 5% w/w cream is an antiseptic cream containing povidone-iodine, used for the treatment and prevention of infection in minor cuts, wounds, scrapes, burns, and various skin infections.
Uses & Indications
Dosage
Adults
Apply a thin layer of Betadine 5% w/w cream to the affected area once or twice daily. May be covered with a sterile dressing.
Elderly
Same as adults. Caution advised for prolonged use on large areas due to potential systemic absorption of iodine.
Renal_impairment
No specific dosage adjustment for topical use. Caution with prolonged use on large areas due to potential for iodine accumulation.
How to Take
For external use only. Clean the affected area, apply a small amount of cream gently, and rub it in. Wash hands before and after application.
Mechanism of Action
Povidone-iodine releases free iodine, which rapidly oxidizes microbial cellular components, leading to cell death. It exhibits broad-spectrum antimicrobial activity against bacteria, viruses, fungi, and protozoa.
Pharmacokinetics
Onset
Rapid onset of antiseptic action.
Excretion
Primarily renal excretion for systemically absorbed iodine.
Half life
Not directly applicable for topical use; systemically absorbed iodine has a short half-life (hours) and is excreted.
Absorption
Minimal systemic absorption through intact skin. Absorption may increase with application to large areas, broken skin, or prolonged use.
Metabolism
Absorbed iodine is metabolized in the body.
Side Effects
Contraindications
- •Hypersensitivity to povidone-iodine or any excipients.
- •Known thyroid disorders (e.g., hyperthyroidism, goiter), especially in large areas or prolonged use.
- •Before or after radioiodine scintigraphy or therapy for thyroid carcinoma.
- •Newborns and infants (up to 6 months of age) due to potential for iodine absorption and thyroid dysfunction.
Drug Interactions
Lithium
Concurrent use, especially with extensive application, may lead to additive thyroid effects due to iodine absorption.
Other Antiseptics
May reduce efficacy or cause skin irritation when used concurrently with other topical antiseptics (e.g., hydrogen peroxide, silver sulfadiazine, mercury-containing products).
Enzyme-based preparations
Povidone-iodine may interact with certain enzyme preparations used for wound cleaning, reducing their effectiveness.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep the tube tightly closed when not in use.
Overdose
Topical overdose is unlikely to cause systemic toxicity unless applied to large areas of broken skin or ingested. Symptoms of systemic iodine toxicity may include thyroid dysfunction, metabolic acidosis, and renal impairment. Management is symptomatic and supportive.
Pregnancy & Lactation
Use with caution and only if clearly needed. High or prolonged systemic absorption of iodine can cross the placenta and be excreted in breast milk, potentially affecting fetal/infant thyroid function. Consult a doctor.
