Description
Overview of the medicine
Betadine 5% w/w Ointment is a broad-spectrum antiseptic used for the prevention and treatment of infections in minor cuts, scrapes, burns, and other superficial wounds.
Uses & Indications
Dosage
Adults
Apply a thin layer of the ointment to the affected area 1 to 3 times daily. The treated area may be covered with a sterile dressing.
Elderly
No specific dosage adjustment is generally required for elderly patients, but caution should be exercised with large or prolonged use.
Renal_impairment
Use with caution in patients with renal impairment, especially with large or prolonged applications, due to potential for systemic iodine absorption.
How to Take
For topical application only. Clean the affected area thoroughly before applying a thin layer of Betadine Ointment. Do not rub in. The area may be covered with a sterile bandage if necessary.
Mechanism of Action
Povidone-iodine releases free iodine upon contact with skin and mucous membranes. This free iodine rapidly kills bacteria, fungi, viruses, protozoa, and spores by oxidizing cellular components, leading to microbial cell death.
Pharmacokinetics
Onset
Rapid antiseptic action, typically within seconds to minutes of application.
Excretion
Absorbed iodine is primarily excreted renally via urine.
Half life
Not well defined for topical application; depends on the extent of systemic absorption. Absorbed iodine has a variable systemic half-life.
Absorption
Systemic absorption from intact skin is minimal. However, significant absorption may occur from large areas of broken skin, open wounds, or mucous membranes.
Metabolism
Absorbed iodine is metabolized in the body, primarily in the thyroid gland if taken up.
Side Effects
Contraindications
- Known hypersensitivity to iodine or povidone
- History of thyroid disorders (e.g., hyperthyroidism, goiter)
- Dermatitis herpetiformis
- Prior to or after radioactive iodine therapy
- In neonates and infants, especially premature infants
Drug Interactions
Lithium
If significant systemic absorption occurs, concurrent use with lithium may lead to additive hypothyroid effects.
Hydrogen peroxide
Incompatible; may reduce the antiseptic efficacy of povidone-iodine.
Silver sulfadiazine
Concurrent use with silver sulfadiazine may inactivate both agents.
Other antiseptics (e.g., those containing mercury)
Should not be used concurrently due to potential for adverse reactions or inactivation.
Storage
Store below 25-30°C. Protect from light and moisture. Keep in the original container. Do not freeze.
Overdose
Systemic absorption of large amounts of povidone-iodine can lead to iodine toxicity, characterized by symptoms such as metallic taste, gastrointestinal upset, burning sensation in the mouth or throat, and thyroid dysfunction. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Use with caution. High doses or prolonged use, especially on large areas of skin, should be avoided during pregnancy and lactation due to the potential for systemic iodine absorption and subsequent fetal/neonatal hypothyroidism. Consult a doctor before use.
