Description
Overview of the medicine
Betadine 7.5% w/v Surgical Scrub is an antiseptic solution used for surgical hand scrubbing and pre-operative skin preparation of patients to reduce the risk of infection.
Uses & Indications
Dosage
Adults
For surgical hand scrub: Wet hands and forearms, apply 5 mL of scrub, spread thoroughly and scrub for 5 minutes. Rinse thoroughly. Apply another 5 mL and scrub for 2.5 minutes, rinse thoroughly. For pre-operative skin prep: Apply undiluted to surgical site, scrub for 2 minutes, rinse off with water and sterile towel.
Elderly
No specific dosage adjustment required.
Renal_impairment
Use with caution in patients with severe renal impairment due to potential for iodine absorption.
How to Take
For external use only. Apply to wet skin, scrub thoroughly, and rinse. Avoid contact with eyes.
Mechanism of Action
Povidone-iodine works by releasing free iodine, which rapidly penetrates microbial cells, oxidizes proteins, nucleotides, and fatty acids, leading to cell death. It has a broad spectrum of antimicrobial activity against bacteria, fungi, viruses, protozoa, and spores.
Pharmacokinetics
Onset
Rapid, within seconds to minutes of application.
Excretion
Absorbed iodide is excreted primarily via the kidneys.
Half life
Not applicable for topical use, as systemic absorption is minimal.
Absorption
Minimal systemic absorption through intact skin. Absorption may increase with application to large areas of broken skin or mucous membranes.
Metabolism
Absorbed iodine is metabolized in the liver, similar to naturally occurring iodide.
Side Effects
Contraindications
- •Known hypersensitivity to iodine or povidone
- •History of thyroid disorders (e.g., hyperthyroidism, goitre)
- •In newborns and infants, especially premature babies, due to risk of iodine-induced hypothyroidism
- •Before and after radioiodine therapy
Drug Interactions
Hydrogen peroxide
Concurrent use may reduce efficacy of both due to inactivation.
Silver sulfadiazine
May reduce efficacy of povidone-iodine and risk of systemic iodine absorption.
Storage
Store below 30°C in a dry place, away from direct sunlight. Keep out of reach of children.
Overdose
Accidental ingestion or extensive topical application to large, broken skin areas may lead to systemic iodine absorption, resulting in symptoms such as metabolic acidosis, renal failure, and thyroid dysfunction (hyperthyroidism or hypothyroidism). Treatment is symptomatic and supportive, including gastric lavage if ingested and monitoring of thyroid function and electrolyte balance.
Pregnancy & Lactation
Povidone-iodine should be used during pregnancy and lactation only if clearly indicated and with caution, as absorbed iodine can cross the placenta and be excreted in breast milk, potentially affecting fetal/infant thyroid function. Short-term, limited application is generally considered low risk.
