Description
Overview of the medicine
Betadine 5% w/v Solution is an antiseptic and disinfectant containing Povidone-iodine. It is used to prevent and treat skin infections in minor cuts, scrapes, and burns. It also helps in sterilizing skin prior to surgical procedures and injections.
Uses & Indications
Dosage
Adults
Apply undiluted solution to the affected area once or twice daily. Allow to dry. May be covered with a sterile dressing.
Elderly
No special dosage adjustment required, use as for adults.
Renal_impairment
Use with caution in patients with renal impairment, especially with prolonged or extensive use, due to potential systemic iodine absorption.
How to Take
For external use only. Apply to skin. Do not ingest. Avoid contact with eyes, ears, and mucous membranes. If contact occurs, rinse thoroughly with water.
Mechanism of Action
Povidone-iodine works by slowly releasing iodine, which is a potent broad-spectrum microbicide. Iodine oxidizes vital cellular components and membranes of microorganisms, leading to their death. It is effective against bacteria, viruses, fungi, and protozoa.
Pharmacokinetics
Onset
Rapid (within seconds) upon application.
Excretion
Mainly renal excretion of iodide.
Half life
Not clinically significant for topical use; systemic half-life of absorbed iodine is approximately 1-2 days.
Absorption
Minimal systemic absorption through intact skin. Absorption may increase with large surface areas, broken skin, or prolonged use.
Metabolism
Absorbed iodine is metabolized in the liver to iodide.
Side Effects
Contraindications
- •Known hypersensitivity to iodine or povidone.
- •Thyroid disorders (e.g., hyperthyroidism, goiter), especially in neonates and young children.
- •Before and after radioiodine therapy.
- •Do not use on large areas of broken skin or for prolonged periods, especially in premature neonates or infants.
- •Severe renal insufficiency (if systemic absorption is a concern).
Drug Interactions
Lithium
May increase the risk of hypothyroidism when used extensively, especially in patients on lithium therapy.
Enzymatic wound cleaners
Povidone-iodine may reduce the effectiveness of enzymatic wound cleaners.
Silver sulfadiazine and hydrogen peroxide
Concomitant use with silver-containing products or hydrogen peroxide may result in inactivation of either agent and/or local discoloration.
Storage
Store below 30°C in a cool, dry place. Protect from light. Keep out of reach of children.
Overdose
Accidental ingestion or extensive topical application on large areas of broken skin can lead to systemic absorption of iodine, potentially causing thyroid dysfunction (hypothyroidism or hyperthyroidism), metabolic acidosis, and renal impairment. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Use with caution. Excessive systemic absorption of iodine may affect fetal thyroid function. Consult a doctor. Short-term, limited application on intact skin is generally considered low risk, but prolonged or widespread use is not recommended.
