Description
Overview of the medicine
Bilergy 20 mg tablet contains Bilastine, a second-generation H1-antihistamine used to treat the symptoms of allergic rhinitis (sneezing, runny nose, nasal itching, itchy eyes, watery eyes) and urticaria (hives, skin rash with itching). It is known for its non-sedating profile.
Uses & Indications
Dosage
Adults
20 mg once daily.
Elderly
No dosage adjustment is required for elderly patients.
Renal_impairment
No dosage adjustment is generally required for patients with mild to moderate renal impairment. Caution is advised in severe renal impairment, though dose adjustment may not be strictly necessary.
How to Take
Take orally, once daily. It should be taken on an empty stomach, one hour before or two hours after food or fruit juice. Swallow the tablet whole with water.
Mechanism of Action
Bilastine is a selective peripheral H1-receptor antagonist. It prevents histamine from binding to H1 receptors, thereby blocking the effects of histamine which cause allergic symptoms like itching, sneezing, and watery eyes. It does not readily cross the blood-brain barrier, contributing to its non-sedating profile.
Pharmacokinetics
Onset
Within 30-60 minutes.
Excretion
Mainly excreted unchanged, with approximately 33% in urine and 67% in feces via bile.
Half life
Approximately 6-12 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration (Cmax) is reached in approximately 1.3 hours. Food and fruit juices significantly reduce oral bioavailability.
Metabolism
Bilastine is minimally metabolized in humans; approximately 95% of the dose is excreted unchanged. It is not a substrate or inhibitor of CYP450 enzymes.
Side Effects
Contraindications
- Hypersensitivity to the active substance (Bilastine) or to any of the excipients.
- Patients with severe renal impairment (use with caution).
Drug Interactions
Diltiazem
May increase plasma concentration of Bilastine.
Ketoconazole and Erythromycin
Increase the plasma concentration of Bilastine. Administer with caution.
P-glycoprotein inhibitors (e.g., Cyclosporine, Ritonavir)
May increase Bilastine plasma levels, requiring caution.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be administered. No specific antidote for Bilastine is known. Gastric lavage and activated charcoal may be considered within the first few hours.
Pregnancy & Lactation
Pregnancy Category B. Limited data from studies in pregnant women. Use during pregnancy only if clearly needed and after consulting a doctor. It is unknown whether Bilastine is excreted in human milk; use with caution during lactation.
