Description
Overview of the medicine
Bronkolax 2 mg syrup contains Salbutamol, a fast-acting bronchodilator used to relieve symptoms of asthma and chronic obstructive pulmonary disease (COPD).
Uses & Indications
Dosage
Adults
Adults: 2 mg or 4 mg (5 ml or 10 ml) three or four times daily. A single dose of up to 20 ml (8 mg) may be given. Max 32 mg/day.
Elderly
Elderly: Initial dose 2 mg (5 ml) three or four times daily. Dosage may be increased to 4 mg (10 ml) three or four times daily if required.
Renal_impairment
No dosage adjustment typically required, but monitor response and consider reduced frequency if severe impairment.
How to Take
Take orally with or without food. Use a calibrated measuring spoon or cup provided with the syrup for accurate dosing. Shake the bottle well before each use.
Mechanism of Action
Salbutamol is a selective beta2-adrenergic agonist. It stimulates beta2-adrenergic receptors in the smooth muscle of the bronchi, leading to relaxation of bronchial smooth muscle and bronchodilation.
Pharmacokinetics
Onset
Bronchodilation typically occurs within 5-15 minutes after oral administration.
Excretion
Mainly excreted in urine within 72 hours as unchanged drug and its sulfate metabolite.
Half life
The plasma half-life of oral salbutamol is approximately 3.8 to 5 hours.
Absorption
Rapidly and well absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Undergoes significant first-pass metabolism in the liver and gastrointestinal wall, primarily to an inactive sulfate conjugate.
Side Effects
Contraindications
- Hypersensitivity to salbutamol or any component of the formulation.
- Threatened abortion during the first or second trimester.
- Use as a sole agent in patients with severe asthma or unstable asthma.
Drug Interactions
Digoxin
May decrease digoxin serum levels.
Diuretics (e.g., Furosemide)
May cause hypokalemia, increasing the risk of cardiac arrhythmias. Monitor potassium levels.
Beta-blockers (e.g., Propranolol)
May reduce the effect of salbutamol and cause severe bronchospasm. Concomitant use is generally contraindicated.
Monoamine Oxidase Inhibitors (MAOIs) / Tricyclic Antidepressants (TCAs)
Increased risk of cardiovascular side effects. Use with caution.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children. Do not freeze.
Overdose
Symptoms of overdose include tachycardia, tremors, palpitations, headache, hypokalemia, and hyperglycemia. Treatment is symptomatic and supportive, usually involving discontinuation of the drug and careful monitoring of vital signs and potassium levels.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Salbutamol is excreted in breast milk; use with caution in lactating mothers and weigh benefits against risks.
