Description
Overview of the medicine
Budesonide nasal spray is a corticosteroid used to treat symptoms of allergic and non-allergic rhinitis, and to manage nasal polyps.
Uses & Indications
Dosage
Adults
Initial dose: 100 mcg (one spray) in each nostril once daily, or 50 mcg (one spray) in each nostril twice daily. Max 200 mcg/day per nostril. Maintenance dose should be the lowest effective dose.
Elderly
No specific dosage adjustment required, but caution is advised in elderly patients with comorbidities.
Renal_impairment
No dosage adjustment necessary.
How to Take
For intranasal use only. Shake the bottle gently before use. Clear nasal passages by blowing nose. Tilt head slightly forward, insert nozzle into nostril, and spray while sniffing gently. Repeat for the other nostril.
Mechanism of Action
Budesonide is a potent glucocorticoid with strong topical anti-inflammatory action. It reduces inflammation, swelling, and mucus production in the nasal passages by inhibiting the release of inflammatory mediators.
Pharmacokinetics
Onset
Clinical effects typically observed within hours, with full therapeutic benefit achieved within a few days to a week.
Excretion
Metabolites are primarily excreted via the urine and feces.
Half life
Approximately 2-3 hours for systemic budesonide.
Absorption
Low systemic bioavailability (approximately 6-20%) due to extensive first-pass metabolism in the liver.
Metabolism
Primarily metabolized in the liver by the cytochrome P450 3A4 (CYP3A4) isoenzyme.
Side Effects
Contraindications
- Hypersensitivity to budesonide or any component of the formulation.
- Untreated localized infection involving the nasal mucosa (e.g., herpes simplex).
Drug Interactions
Strong CYP3A4 inhibitors (e.g., Ketoconazole, Itraconazole, Ritonavir, Atazanavir)
May significantly increase systemic exposure to budesonide, potentially leading to Cushing's syndrome and adrenal suppression. Concurrent use should be avoided or dose adjusted.
Storage
Store upright at room temperature (15°C-30°C / 59°F-86°F). Do not freeze. Keep out of reach of children.
Overdose
Acute overdose with budesonide nasal spray is unlikely to produce serious systemic effects due to its low systemic bioavailability. Chronic excessive dosing may lead to signs and symptoms of hypercorticism.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Budesonide is excreted in human milk; caution should be exercised when administered to a nursing woman.
