Description
Overview of the medicine
Buticef 90 mg Suspension contains Cefixime, which is a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections in children.
Uses & Indications
Dosage
Elderly
No specific dose adjustment is required for elderly patients unless they have severe renal impairment.
Children
For children aged 6 months to 12 years: 8 mg/kg/day administered as a single dose or in two divided doses. Duration of treatment usually 5-10 days, depending on the infection. Maximum daily dose should not exceed 400 mg.
Renal_impairment
Dosage adjustment is recommended for patients with creatinine clearance less than 60 mL/min. For moderate impairment (20-49 mL/min), the dose should be reduced by 25%. For severe impairment (< 20 mL/min), the dose should be reduced by 50%.
How to Take
Buticef 90 mg Suspension should be taken orally, with or without food. Shake the bottle well before each use to ensure uniform dosage. Use the provided measuring cup or syringe for accurate dosing.
Mechanism of Action
Cefixime exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) which leads to inhibition of the transpeptidation step of peptidoglycan synthesis, thereby disrupting cell wall formation and causing bacterial cell lysis.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved 2-6 hours after administration.
Excretion
Approximately 50% is excreted unchanged in the urine over 24 hours, and some is excreted via biliary/fecal route.
Half life
Plasma half-life averages 3-4 hours.
Absorption
Approximately 40-50% orally absorbed, independent of food.
Metabolism
Not extensively metabolized in the liver; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- History of anaphylactic reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Warfarin
May increase prothrombin time and INR, leading to increased bleeding risk. Close monitoring of coagulation parameters is advised.
Carbamazepine
Concomitant use may increase carbamazepine plasma levels. Monitor carbamazepine levels.
Oral Contraceptives
May reduce the efficacy of oral contraceptives. Advise patients to use an additional non-hormonal birth control method.
Aminoglycosides / Loop Diuretics
May increase the risk of nephrotoxicity.
Storage
Store the dry powder below 30°C in a dry place, away from direct sunlight and moisture. Keep the reconstituted suspension refrigerated (2-8°C) and discard any unused portion after 14 days.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Animal reproduction studies have shown no evidence of harm to the fetus. However, use in pregnancy should be only if clearly needed. Cefixime is excreted in human milk in small amounts; use with caution during lactation.
