Description
Overview of the medicine
Cefaz 2 gm Injection contains Cefazolin, a first-generation cephalosporin antibiotic used to treat a variety of bacterial infections and for surgical prophylaxis. It is administered intravenously or intramuscularly.
Uses & Indications
Dosage
Adults
Typical adult dose for moderate to severe infections is 500 mg to 1 gm every 6 to 8 hours. For surgical prophylaxis, 1 gm IV/IM 30-60 minutes prior to surgery, then 500 mg to 1 gm every 6-8 hours for 24 hours.
Elderly
Dose adjustment may be necessary in elderly patients with impaired renal function.
Renal_impairment
Dosage must be adjusted based on creatinine clearance.
How to Take
For IV administration, reconstitute with sterile water for injection and administer slowly over 3-5 minutes or infuse over 30-60 minutes. For IM administration, reconstitute with sterile water or lidocaine and inject deep into a large muscle mass.
Mechanism of Action
Cefazolin is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death.
Pharmacokinetics
Onset
Rapid, typically within minutes of IV administration.
Excretion
Primarily via renal excretion (glomerular filtration and tubular secretion).
Half life
Approximately 1.8 hours.
Absorption
Rapidly and completely absorbed after intramuscular (IM) administration; high bioavailability.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to cefazolin or any other cephalosporin antibiotics
- History of severe hypersensitivity reaction to penicillin (due to potential cross-reactivity)
Drug Interactions
Probenecid
Increases and prolongs cefazolin plasma levels by decreasing renal tubular secretion.
Anticoagulants (e.g., Warfarin)
May enhance anticoagulant effect; monitor INR.
Nephrotoxic drugs (e.g., Aminoglycosides, Furosemide)
Increased risk of nephrotoxicity.
Storage
Store powder for injection at controlled room temperature (20-25°C) away from moisture and light. Reconstituted solutions should be stored as per specific instructions, usually refrigerated.
Overdose
Symptoms of overdose may include seizures, neurological toxicity. Management involves supportive care and hemodialysis if severe renal impairment exists.
Pregnancy & Lactation
Pregnancy Category B. Cefazolin crosses the placenta. Use in pregnancy only if clearly needed. Excreted in breast milk in low concentrations; caution advised during lactation.

YesClinical Trials
Extensive clinical trials have established cefazolin's efficacy and safety for indicated bacterial infections and surgical prophylaxis since its introduction.
Lab Monitoring
- Renal function tests (e.g., creatinine, BUN) during prolonged therapy or in patients with renal impairment
- Complete blood count (CBC) during prolonged therapy
- Liver function tests (LFTs) if abnormalities are suspected
Doctor Notes
- Careful history taking regarding penicillin allergy is crucial due to potential cross-reactivity.
- Monitor renal function in patients receiving high doses or with pre-existing renal impairment.
- Administer slowly to avoid vein irritation with IV administration.
Patient Guidelines
- Report any signs of allergic reaction immediately.
- Complete the full course of treatment even if symptoms improve.
- Inform your doctor about all other medications you are taking.
- Report any persistent diarrhea or severe stomach cramps.
Missed Dose Advice
If a dose is missed, administer it as soon as possible. If it is almost time for the next dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose.
Driving Precautions
Cefazolin is generally not expected to impair the ability to drive or operate machinery. However, if side effects like dizziness occur, advise caution.
Lifestyle Advice
- Maintain good hygiene to prevent further infections.
- Stay hydrated.
- Avoid self-medication.
