Uses & Indications

Spasms of smooth muscles associated with diseases of the biliary tract (e.g., cholecystolithiasis, cholecystitis, cholangiolithiasis, papillitis)

Spasms of smooth muscles in the urinary tract (e.g., nephrolithiasis, ureterolithiasis, pyelitis, cystitis, bladder tenesmus)

Spasms of gastrointestinal smooth muscles (e.g., gastric and duodenal ulcer, gastritis, enteritis, colitis, spastic constipation, irritable bowel syndrome)

Tension headache (off-label use)

Dosage

Adults

Typically 40-80 mg (2-4 mL) IM or IV, 1-3 times daily. Max daily dose 240 mg.

Elderly

Similar to adult dosage, but caution may be advised in patients with underlying comorbidities. Dose adjustment might not be strictly necessary based on age alone.

Renal_impairment

No specific dose adjustment is generally required, but caution is advised in severe renal impairment.

How to Take

For intravenous (IV) or intramuscular (IM) administration. IV administration should be slow (over 3-5 minutes) to avoid hypotension. It can be diluted with normal saline.

Drotaverine acts directly on smooth muscle by inhibiting phosphodiesterase (PDE) type IV, leading to an increase in intracellular cAMP. This increased cAMP causes relaxation of smooth muscle cells, thereby relieving spasms.
