Description
Overview of the medicine
Colipan 20 mg Injection contains Drotaverine Hydrochloride, a potent antispasmodic agent. It is used to relieve smooth muscle spasms in various conditions, offering rapid action due to its injectable form.
Uses & Indications
Dosage
Adults
Typically 40-80 mg (2-4 mL) IM or IV, 1-3 times daily. Max daily dose 240 mg.
Elderly
Similar to adult dosage, but caution may be advised in patients with underlying comorbidities. Dose adjustment might not be strictly necessary based on age alone.
Renal_impairment
No specific dose adjustment is generally required, but caution is advised in severe renal impairment.
How to Take
For intravenous (IV) or intramuscular (IM) administration. IV administration should be slow (over 3-5 minutes) to avoid hypotension. It can be diluted with normal saline.
Mechanism of Action
Drotaverine acts directly on smooth muscle by inhibiting phosphodiesterase (PDE) type IV, leading to an increase in intracellular cAMP. This increased cAMP causes relaxation of smooth muscle cells, thereby relieving spasms.
Pharmacokinetics
Onset
Within 2-5 minutes after IV administration, 10-15 minutes after IM administration.
Excretion
Primarily excreted via urine and feces as metabolites.
Half life
Approximately 2-2.5 hours.
Absorption
Rapidly and completely absorbed after IM/IV administration.
Metabolism
Extensively metabolized in the liver, mainly by O-demethylation.
Side Effects
Contraindications
- Hypersensitivity to Drotaverine or any excipients
- Severe hepatic, renal or cardiac insufficiency
- Children under 1 year
- AV block II-III degree
- Cardiogenic shock
- Severe hypotension
Drug Interactions
Levodopa
May decrease the anti-Parkinsonian effect of Levodopa due to its PDE inhibiting activity. Avoid concomitant use or adjust Levodopa dose.
Benzodiazepines
Additive sedative effects if combined.
Other antispasmodics
Enhanced antispasmodic effect when used with other antispasmodic agents.
Storage
Store below 30°C. Protect from light. Do not freeze. Keep out of the reach of children.
Overdose
Symptoms may include cardiac conduction disturbances, decreased myocardial contractility, and severe hypotension. Management is symptomatic and supportive, including gastric lavage if ingested, and monitoring of vital signs.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed and potential benefits outweigh risks. Drotaverine is excreted into breast milk in animals, but human data is limited. Use with caution during lactation.
