Description
Overview of the medicine
Dexlion 60 mg capsule is an extended-release proton pump inhibitor (PPI) used to decrease the amount of acid produced in the stomach. It is primarily used to treat conditions like gastroesophageal reflux disease (GERD) and erosive esophagitis.
Uses & Indications
Dosage
Adults
For healing of EE: 60 mg once daily for up to 8 weeks. For maintenance of healed EE and relief of heartburn: 30 mg once daily. For symptomatic non-erosive GERD: 30 mg once daily for 4 weeks.
Elderly
No dosage adjustment is necessary for elderly patients.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Administer orally, once daily, with or without food. The capsule should be swallowed whole. Do not chew or crush the capsule. For patients who have difficulty swallowing, the capsule can be opened and the intact granules sprinkled onto a tablespoon of applesauce and swallowed immediately.
Mechanism of Action
Dexlansoprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase pump (proton pump) in the gastric parietal cells, thereby blocking the final step of gastric acid secretion.
Pharmacokinetics
Onset
Within 1-2 hours of administration.
Excretion
Approximately 50% of the administered dose is excreted in the urine and 50% in the feces.
Half life
Plasma elimination half-life is approximately 1-2 hours, but its duration of action is much longer due to irreversible binding to the proton pump.
Absorption
Dexlansoprazole is a dual delayed-release formulation designed to provide two distinct releases of medication. It is absorbed from the small intestine.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 (CYP) enzymes CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Known hypersensitivity to dexlansoprazole or any component of the formulation.
- Concomitant use with rilpivirine-containing products.
Drug Interactions
Warfarin
Co-administration may result in increased INR and prothrombin time; monitor for increased bleeding risk.
Clopidogrel
Reduced plasma concentrations of the active metabolite of clopidogrel; consider alternative anti-platelet therapy.
Methotrexate
Concomitant use with PPIs may increase and prolong serum concentrations of methotrexate and/or its metabolite, potentially leading to toxicity.
Drugs requiring acidic pH for absorption (e.g., Ketoconazole, Ampicillin esters, Iron salts, Digoxin)
Dexlansoprazole decreases gastric acidity, which may reduce the absorption of these drugs.
Storage
Store below 30°C (86°F). Protect from moisture and light. Keep out of reach of children.
Overdose
Experience with dexlansoprazole overdose is limited. Management of overdose should be symptomatic and supportive. Dexlansoprazole is not removed from the circulation by hemodialysis.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known whether dexlansoprazole is excreted in human milk; caution should be exercised when administered to a nursing mother.
