Description
Overview of the medicine
Domperidone is a dopamine antagonist used to treat nausea, vomiting, and gastrointestinal motility disorders. It works by blocking dopamine receptors in the central nervous system and the gut.
Uses & Indications
Dosage
Adults
10-20 mg (2-4 teaspoons of 5 mg/5 mL suspension) three to four times daily, 15-30 minutes before meals and at bedtime. Maximum daily dose: 80 mg.
Elderly
Start with a lower dose (e.g., 10 mg three times daily) and monitor for adverse effects.
Renal_impairment
In severe renal impairment (creatinine clearance < 30 mL/min), frequency of dosing should be reduced to once or twice daily, depending on severity.
How to Take
Take orally, preferably 15-30 minutes before meals. Shake the suspension well before use.
Mechanism of Action
Domperidone blocks dopamine D2 receptors in the chemoreceptor trigger zone (CTZ) located outside the blood-brain barrier and in the gastrointestinal tract. This action leads to increased gastric emptying and intestinal peristalsis, and an antiemetic effect.

Side Effects
Contraindications
- Known hypersensitivity to domperidone or any component of the formulation
- Prolactin-secreting pituitary tumor (prolactinoma)
- Moderate to severe hepatic impairment
- Conditions where stimulation of gastric motility is dangerous (e.g., gastrointestinal hemorrhage, mechanical obstruction, perforation)
- Patients with known prolonged QT intervals, significant electrolyte disturbances (hypokalemia, hypomagnesemia), or pre-existing cardiac disease (e.g., congestive heart failure)
- Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, erythromycin, clarithromycin)

Domperidone may antagonize their effects.
Antacids and antisecretory agents (e.g., proton pump inhibitors)
May reduce the oral bioavailability of domperidone if taken concomitantly. Should be taken before domperidone.
Strong CYP3A4 inhibitors (e.g., ketoconazole, erythromycin, clarithromycin)
Increased plasma concentrations of domperidone, leading to a higher risk of QT prolongation and cardiac arrhythmias. Concomitant use is contraindicated.
QT-prolonging drugs (e.g., antiarrhythmics, antipsychotics, antidepressants)
Increased risk of QT prolongation and ventricular arrhythmias. Concomitant use should be avoided or used with extreme caution.
Storage
Store below 30°C in a dry place, protected from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose may include somnolence, disorientation, and extrapyramidal reactions, particularly in children. Treatment is symptomatic and supportive; gastric lavage may be performed, and activated charcoal may be given. Anticholinergic, anti-parkinsonian drugs may be useful for extrapyramidal reactions.
Pregnancy & Lactation
Pregnancy Category C. Studies in animals have shown reproductive toxicity. Not recommended during pregnancy unless the potential benefits outweigh the potential risks. Domperidone is excreted in breast milk in small amounts; use with caution in nursing mothers, especially in infants with cardiac risk factors.
