Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. It is used to treat acid-related conditions like gastroesophageal reflux disease (GERD), peptic ulcers, and to prevent NSAID-induced ulcers.
Uses & Indications
Dosage
Adults
GERD/Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. NSAID-ulcers: 20 mg once daily for 4-8 weeks. H. pylori eradication: 20 mg twice daily for 7-14 days (with antibiotics). Zollinger-Ellison: Initial 40 mg twice daily, adjusted as needed.
Elderly
No dose adjustment generally required. Use with caution in severe hepatic impairment.
Renal_impairment
No dose adjustment required.
How to Take
Take capsule whole, at least one hour before a meal. Do not chew or crush the capsule. For patients with swallowing difficulties, the capsule can be opened, and the pellets mixed with a small amount of water or fruit juice (do not crush pellets), and drunk immediately.
Mechanism of Action
Esomeprazole is an S-isomer of omeprazole. It selectively inhibits the H+/K+-ATPase enzyme system (the proton pump) at the secretory surface of the gastric parietal cells, thereby blocking the final step of acid secretion.

Absorption
Rapidly absorbed after oral administration. Bioavailability increases with repeated doses. Food delays and decreases absorption.

Extensively hepatic metabolism by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Diazepam
May decrease clearance of diazepam (CYP2C19 substrate).
Warfarin
Increased INR/prothrombin time; monitor closely.
Tacrolimus
Increased tacrolimus serum concentrations.
Clopidogrel
May reduce antiplatelet effect of clopidogrel (avoid concomitant use if possible).
Methotrexate
Increased plasma levels of methotrexate (monitor for toxicity).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Symptoms may include drowsiness, tachycardia, nausea, diarrhea, abdominal pain, and headache. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no risk, but human studies are lacking. Use only if clearly needed. Excreted in breast milk, so caution is advised; weigh benefits vs. risks.
