Overview of the medicine
Floxy 500 mg Tablet contains Ciprofloxacin, a broad-spectrum fluoroquinolone antibiotic used to treat a wide variety of bacterial infections.
Uses & Indications
Dosage
Adults
Typically 250 mg to 750 mg orally twice daily, depending on the severity and type of infection. For Floxy 500 mg, common dose is 500 mg twice daily.
Elderly
No specific dose adjustment is needed based on age alone, but renal function should be considered. Use lower end of adult dose range if renal impairment is present.
Renal_impairment
Dose adjustment is necessary. For creatinine clearance 30-60 mL/min, 250-500 mg every 12 hours. For <30 mL/min, 250-500 mg every 18-24 hours.
How to Take
Take orally with or without food. Do not take with dairy products or calcium-fortified juices. Ensure adequate hydration. Swallow the tablet whole with water.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes necessary for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and cell death.

Onset
Within 1-2 hours for antibacterial action.
Excretion
Primarily excreted unchanged by the kidneys (50-70%) and a smaller portion through feces.
Half life
Approximately 3-5 hours.
Absorption
Well absorbed from the gastrointestinal tract (bioavailability 70-80%). Peak plasma concentrations are achieved 1-2 hours after oral administration.
Metabolism
Partially metabolized in the liver to four active metabolites (less potent than ciprofloxacin).
Side Effects
Contraindications
- Hypersensitivity to ciprofloxacin or other fluoroquinolones.
- Concomitant administration of tizanidine.
- Children and adolescents (except in specific conditions like cystic fibrosis or anthrax exposure).
Drug Interactions
Caffeine
Decreased clearance of caffeine, potentially leading to increased caffeine effects.
Warfarin
Increased anticoagulant effect, monitor INR.
Tizanidine
Contraindicated due to significant increase in tizanidine plasma concentrations, leading to hypotension and somnolence.
Theophylline
Increased plasma levels of theophylline, monitor for toxicity.
Antacids (containing Mg, Al), Iron, Zinc, Sucralfate, Didanosine
Reduced absorption of ciprofloxacin. Administer ciprofloxacin 2 hours before or 6 hours after these agents.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In the event of acute overdosage, reversible renal toxicity has been reported. Gastric emptying (e.g., vomiting or gastric lavage) may be considered. Maintain adequate hydration. Symptomatic and supportive treatment is recommended. Hemodialysis or peritoneal dialysis remove only a small amount of ciprofloxacin.
Pregnancy & Lactation
Ciprofloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk; therefore, caution should be exercised when administering to a nursing mother. Generally not recommended.
