Overview of the medicine
Fulvestrant is a hormonal therapy used for the treatment of hormone receptor-positive, human epidermal growth factor receptor 2-negative advanced or metastatic breast cancer in postmenopausal women.
Uses & Indications
Dosage
Adults
500 mg intramuscularly on Days 1, 15, 29, and once monthly thereafter. (Administered as two 250 mg/5 mL injections).
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
Mild-to-moderate renal impairment: No dosage adjustment. Severe renal impairment: Use with caution.
How to Take
Administer as a deep intramuscular injection, typically into the buttock. Administer slowly (1-2 minutes per injection). Avoid intravascular injection.
Mechanism of Action
Fulvestrant is a selective estrogen receptor downregulator (SERD). It binds to the estrogen receptor (ER) with high affinity and competitively inhibits estrogen binding. It also leads to the degradation of the ER protein, thus decreasing ER levels in target tissues.

Gradual onset of action; clinical effects are observed over several weeks.
Excretion
Primarily excreted in feces (approximately 90%) after hepatic metabolism; less than 1% renal excretion.
