Description
Overview of the medicine
Hectamox 125 mg Pediatric Drop is an antibiotic medicine containing Amoxicillin, used to treat a wide range of bacterial infections in children, including respiratory tract infections, ear infections, skin infections, and urinary tract infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Not typically used in pediatric drop form for adults. Adults usually take higher strength tablets/capsules.
Elderly
Not typically used in pediatric drop form for elderly. Dosage adjustments may be needed based on renal function for higher strength forms.
Renal_impairment
For severe renal impairment (CrCl < 30 mL/min), dosage reduction or extended dosing interval may be necessary. Consult a physician.
How to Take
Administer orally. Shake the bottle well before each use. Use the provided dropper or measuring spoon to ensure accurate dosing. Can be taken with or without food. Complete the full course of treatment as prescribed, even if symptoms improve.
Mechanism of Action
Amoxicillin is a beta-lactam antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). This disrupts the peptidoglycan layer, leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Antibacterial effects typically begin within 30-60 minutes after administration.
Excretion
Primarily excreted unchanged by the kidneys via glomerular filtration and tubular secretion. About 60-80% of a dose is excreted within 6 hours.
Half life
Approximately 1-1.5 hours in adults with normal renal function; longer in neonates and individuals with renal impairment.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, especially when administered orally. Peak plasma concentrations are achieved within 1-2 hours.
Metabolism
Minimally metabolized; approximately 10-25% is metabolized to inactive penicilloic acid.
Side Effects
Contraindications
- Known hypersensitivity to Amoxicillin, any other penicillin, or any component of the formulation
- History of severe hypersensitivity reaction to another beta-lactam antibiotic (e.g., cephalosporins)
Drug Interactions
Warfarin
May prolong prothrombin time; monitor INR closely.
Probenecid
Increases amoxicillin plasma levels by decreasing renal tubular secretion.
Methotrexate
Increased methotrexate toxicity due to reduced renal excretion.
Oral contraceptives
May reduce efficacy of oral contraceptives (though clinical significance is debatable).
Storage
Store dry powder below 25°C, protected from moisture. Keep the reconstituted suspension refrigerated (2-8°C). Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and rarely, convulsions. Treatment is generally supportive and symptomatic. Gastric lavage or activated charcoal may be considered if ingestion is recent. Hemodialysis can remove amoxicillin from circulation.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe for use during pregnancy when clearly needed. Small amounts are excreted in breast milk; generally considered compatible with breastfeeding, but monitor infants for diarrhea, candidiasis, or allergic reactions.

Lab Monitoring
- Periodically monitor renal, hepatic, and hematopoietic function during prolonged therapy. Monitor for signs of superinfection.
Doctor Notes
- Counsel parents/caregivers on the importance of completing the full course to prevent resistance.
- Advise on proper reconstitution and administration technique for pediatric drops.
- Be vigilant for signs of allergic reactions, especially after the first dose.
