Overview of the medicine
Lesterol 20 mg Capsule contains Atorvastatin, a medication used to lower cholesterol and triglycerides in the blood. It helps prevent heart attacks, strokes, and other heart-related problems.
Uses & Indications
Dosage
Adults
Initial dose 10-20 mg once daily. Max 80 mg daily. Dosage should be individualized based on LDL-C levels.
Elderly
No dosage adjustment needed, but use with caution.
Renal_impairment
No dosage adjustment needed.
How to Take
Take orally once daily, with or without food, at any time of day, preferably in the evening.
Mechanism of Action
Atorvastatin is a selective, competitive inhibitor of HMG-CoA reductase, the enzyme that catalyzes the rate-limiting step in cholesterol biosynthesis. It decreases hepatic cholesterol synthesis and increases the number of LDL receptors on the hepatocyte surface, enhancing LDL uptake and catabolism.

Onset
Lipid-lowering effects begin within 2 weeks, maximal effect in 2-4 weeks.
Excretion
Primarily excreted in bile following hepatic metabolism. Less than 2% is excreted in urine.
Half life
Approximately 14 hours (due to active metabolites, half-life of HMG-CoA reductase inhibitory activity is 20-30 hours).
Absorption
Rapidly absorbed, peak plasma concentrations reached within 1-2 hours.
Metabolism
Extensively metabolized by cytochrome P450 3A4 (CYP3A4) to active and inactive metabolites.
Side Effects
Contraindications
- Active liver disease or unexplained persistent elevations of serum transaminases
- Pregnancy and lactation
- Hypersensitivity to atorvastatin or any component of the formulation
Drug Interactions
Warfarin
May enhance anticoagulant effect.
Cyclosporine, Gemfibrozil, Niacin
Increased risk of myopathy/rhabdomyolysis.
Clarithromycin, Itraconazole, Protease inhibitors
Increased plasma concentrations of atorvastatin.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Treatment should be symptomatic and supportive. Hemodialysis is unlikely to significantly enhance atorvastatin clearance.
Pregnancy & Lactation
Contraindicated during pregnancy and breastfeeding due to potential fetal harm and excretion in breast milk. Women of childbearing potential should use adequate contraception.
