Description
Overview of the medicine
Levamdocal 125 mg Tablet is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain, reduce inflammation, and lower fever. It is typically prescribed for conditions such as arthritis, musculoskeletal pain, and dysmenorrhea.
Uses & Indications
Dosage
Adults
Oral: 125 mg once or twice daily, or as directed by physician. Maximum daily dose should not exceed 250 mg.
Elderly
A lower initial dose (e.g., 125 mg once daily) may be considered, with careful monitoring for adverse effects, especially renal and gastrointestinal.
Renal_impairment
Use with caution. Dosage reduction may be necessary in patients with moderate to severe renal impairment (CrCl <30 mL/min). Avoid in severe renal failure.
How to Take
Take the tablet whole with a glass of water, preferably with food or milk to minimize gastrointestinal discomfort. Do not crush or chew.
Mechanism of Action
Levamdocal works by inhibiting the activity of cyclooxygenase (COX) enzymes, primarily COX-2, which are responsible for the synthesis of prostaglandins. Prostaglandins are mediators of pain, inflammation, and fever.
Pharmacokinetics
Onset
Pain relief typically starts within 30-60 minutes.
Excretion
Mainly excreted in urine (about 60-70%) as metabolites, with some excretion in feces (about 30-40%).
Half life
Approximately 6-8 hours.
Absorption
Rapidly and well absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are usually reached within 1-2 hours.
Metabolism
Extensively metabolized in the liver, primarily via cytochrome P450 enzymes (e.g., CYP2C9), to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Levamdocal or any NSAIDs (e.g., aspirin, ibuprofen)
- History of gastrointestinal bleeding or peptic ulcer disease
- Severe renal or hepatic impairment
- Severe heart failure
- Third trimester of pregnancy
- Coronary Artery Bypass Graft (CABG) surgery (peri-operative pain)
Drug Interactions
Lithium
Increased plasma lithium levels, leading to toxicity.
Methotrexate
Increased methotrexate toxicity.
Corticosteroids
Increased risk of gastrointestinal ulceration and bleeding.
ACE Inhibitors/ARBs
Reduced antihypertensive effect, increased risk of renal impairment.
Diuretics (e.g., Furosemide)
Reduced diuretic and antihypertensive effects, increased risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. In severe cases, acute renal failure, coma, and convulsions may occur. Treatment is symptomatic and supportive. Gastric lavage or activated charcoal may be considered if ingested recently.
Pregnancy & Lactation
Pregnancy Category C in early pregnancy, changing to Category D in the third trimester (due to risk of premature closure of the fetal ductus arteriosus). Should be avoided in the third trimester. Use with caution during lactation; small amounts may pass into breast milk, consult a doctor.
