Overview of the medicine
Moxin 500 mg Capsule is a broad-spectrum penicillin antibiotic used to treat a variety of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Typical dose is 250-500 mg every 8 hours, or 500-875 mg every 12 hours, depending on the severity and type of infection. For severe infections, up to 1 gram every 8 hours.
Elderly
No specific dose adjustment for elderly with normal renal function. Dose adjustment needed for renal impairment.
Renal_impairment
Creatinine clearance 10-30 mL/min: 250-500 mg every 12 hours. Creatinine clearance <10 mL/min: 250-500 mg every 24 hours.
How to Take
Take the capsule orally with a glass of water, with or without food. Complete the full course of treatment as prescribed by your doctor, even if you feel better.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). This leads to osmotic instability and bacterial cell lysis.
Pharmacokinetics
Onset
Peak plasma concentrations reached within 1-2 hours.
Excretion
Primarily excreted unchanged in the urine by glomerular filtration and tubular secretion. Approximately 60% of an oral dose is excreted within 6-8 hours.
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Rapidly and well-absorbed (75-90%) from the gastrointestinal tract, not significantly affected by food.
Metabolism
Minimal metabolism; approximately 10-25% is metabolized to inactive penicilloic acid.
Side Effects
Contraindications
- •Hypersensitivity to amoxicillin or any other penicillin antibiotic
- •History of severe allergic reaction to any beta-lactam antibiotic (e.g., cephalosporins)
Drug Interactions
Probenecid
Decreases renal tubular secretion of amoxicillin, resulting in increased and prolonged plasma levels of amoxicillin.
Allopurinol
Increased risk of skin rash.
Methotrexate
Reduced renal tubular secretion of methotrexate, leading to increased methotrexate levels and potential toxicity.
Oral contraceptives
May reduce the efficacy of oral contraceptives (though evidence is weak, advise additional contraception).
Oral anticoagulants (e.g., Warfarin)
May increase the anticoagulant effect, requiring monitoring of INR.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Overdose symptoms may include gastrointestinal symptoms (nausea, vomiting, diarrhea) and disturbances of fluid and electrolyte balance. Renal impairment has been reported in a small number of cases. Treatment is symptomatic and supportive, with forced diuresis or hemodialysis if necessary.
Pregnancy & Lactation
Pregnancy Category B. Amoxicillin is generally considered safe during pregnancy. It is excreted in small amounts in breast milk, but generally considered compatible with breastfeeding. Consult your doctor.
