Description
Overview of the medicine
Moxifloxacin is a broad-spectrum fluoroquinolone antibiotic used to treat various bacterial infections. It works by stopping the growth of bacteria. This 500 mg injection is typically administered intravenously for severe infections.
Uses & Indications
Dosage
Adults
The usual dose is 500 mg administered once daily by intravenous infusion. The duration of treatment depends on the type and severity of infection.
Elderly
No dosage adjustment is required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with mild to moderate renal impairment. For severe renal impairment or end-stage renal disease (including patients on dialysis), no dosage adjustment is specified, but caution is advised, and patients should be carefully monitored.
How to Take
Administer by intravenous infusion over a period of 60 minutes. Do not administer as a rapid or bolus injection. Compatible with commonly used infusion solutions.
Mechanism of Action
Moxifloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and cell death.
Pharmacokinetics
Onset
Rapid onset after intravenous infusion.
Excretion
Approximately 45% of the dose is excreted as unchanged drug and metabolites via the feces and 52% via urine.
Half life
Approximately 11-15 hours.
Absorption
Rapid and complete absorption after intravenous administration, with 100% bioavailability.
Metabolism
Primarily hepatic via glucuronide and sulfate conjugation (inactive metabolites).
Side Effects
Contraindications
- •Hypersensitivity to moxifloxacin or other quinolone antibiotics.
- •History of tendon disorders or rupture associated with quinolone use.
- •Patients with congenital or documented acquired QT prolongation.
- •Uncorrected hypokalemia or hypomagnesemia.
- •History of clinically significant bradycardia, heart failure with reduced left ventricular ejection fraction, or history of symptomatic arrhythmias.
- •Concomitant administration with other QT-prolonging drugs.
Drug Interactions
Warfarin
Enhanced anticoagulant effect, requiring close monitoring of INR (International Normalized Ratio).
Corticosteroids
Increased risk of tendinitis and tendon rupture, especially in elderly patients and those with kidney, heart, or lung transplants.
QT-prolonging drugs
Avoid co-administration with Class IA and III antiarrhythmics, tricyclic antidepressants, macrolides, and antipsychotics due to increased risk of QT prolongation.
Storage
Store below 30°C. Do not freeze. Protect from light. Keep out of the reach of children.
Overdose
In the event of an acute overdose, symptomatic and supportive treatment should be initiated. ECG monitoring is advised due to the risk of QT prolongation. There is no specific antidote. Hemodialysis or peritoneal dialysis remove only a small amount of moxifloxacin.
Pregnancy & Lactation
Pregnancy Category C. Moxifloxacin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human breast milk; therefore, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
