Overview of the medicine
Napa 10 mg injection contains Paracetamol (Acetaminophen), a widely used medication for fast and effective relief of pain and fever. It is administered intravenously, suitable for situations where oral administration is not possible or rapid onset of action is required.
Uses & Indications
Dosage
Adults
Usually 1000 mg (1 vial) administered as a 15-minute intravenous infusion, up to 4 times a day. Maximum daily dose: 4000 mg.
Elderly
Similar to adult dosage, but caution with reduced renal function. Max 3000 mg/day if severe renal impairment.
Renal_impairment
Creatinine clearance 10-50 mL/min: Increase interval to 6 hours. Creatinine clearance <10 mL/min: Increase interval to 8 hours. Max daily dose depends on severity.
How to Take
Administer as a 15-minute intravenous infusion. Do not administer by rapid bolus injection.
Mechanism of Action
Paracetamol is thought to act primarily in the central nervous system, inhibiting prostaglandin synthesis, which leads to its analgesic and antipyretic effects. It exhibits weak anti-inflammatory activity.
Pharmacokinetics
Onset
5-10 minutes for analgesia, within 30 minutes for antipyresis.
Excretion
Mainly renal, as glucuronide and sulfate conjugates, with less than 5% excreted as unchanged drug.
Half life
1.5-3 hours in adults, slightly shorter in children.
Absorption
Rapid and complete absorption after intravenous administration.
Metabolism
Primarily hepatic, conjugated with glucuronic acid and sulfate. A small portion is metabolized by cytochrome P450 to a toxic intermediate (NAPQI), rapidly detoxified by glutathione.
Side Effects
Contraindications
- •Hypersensitivity to paracetamol or propacetamol (a prodrug of paracetamol) or any excipients.
- •Severe hepatocellular insufficiency or active liver disease.
Drug Interactions
Alcohol
Chronic alcohol abuse increases risk of hepatotoxicity.
Warfarin
May enhance the anticoagulant effect, increasing risk of bleeding.
Phenytoin, Phenobarbital, Carbamazepine
May increase the risk of hepatotoxicity with paracetamol overdose.
Storage
Store below 30°C, protected from light. Do not refrigerate or freeze.
Overdose
Symptoms include nausea, vomiting, abdominal pain, pallor, and anorexia, typically within 24 hours. Severe liver damage may occur. Treatment involves administration of N-acetylcysteine (NAC) and supportive care.
Pregnancy & Lactation
Paracetamol can be used during pregnancy and lactation if clinically needed, but at the lowest effective dose and for the shortest possible duration. Consult a doctor.
