Description
Overview of the medicine
Napa 250 mg suppository contains paracetamol, a common pain reliever and fever reducer. It is administered rectally and is often used in children or patients who cannot take oral medication.
Uses & Indications
Dosage
Adults
Not typically recommended for adults, as this is a pediatric strength. If adult use is considered, 500-1000 mg every 4-6 hours, maximum 4g/day (using appropriate strength).
Elderly
Not typically recommended for elderly. Consult a physician for appropriate strength and dosage.
Pediatric
12.5 mg/kg per dose, every 4-6 hours, not exceeding 5 doses in 24 hours. For children weighing 17-25 kg (approx 6-9 years): 1 suppository (250 mg) every 4-6 hours.
Renal_impairment
Dosage adjustment necessary. Consult a physician for specific pediatric dosing in renal impairment.
How to Take
For rectal use only. Remove the suppository from its wrapper and insert it pointed end first into the rectum. Lie down for a few minutes to allow it to dissolve. Wash hands before and after use.
Mechanism of Action
Paracetamol's exact mechanism is not fully understood, but it is believed to act primarily by inhibiting prostaglandin synthesis in the central nervous system (CNS), which leads to its analgesic and antipyretic effects. It has weak anti-inflammatory effects.
Pharmacokinetics
Onset
Analgesic: 30-60 minutes; Antipyretic: 30 minutes.
Excretion
Excreted mainly by the kidneys as inactive glucuronide and sulfate conjugates, with a small amount of unmetabolized drug.
Half life
1-4 hours (typically 2-3 hours in adults, slightly longer in neonates).
Absorption
Well absorbed from the rectal route, though peak plasma concentrations may be achieved more slowly and lower than oral administration.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation. A small portion is metabolized by cytochrome P450 to a toxic intermediate (NAPQI), which is then detoxified by glutathione.
Side Effects
Contraindications
- •Known hypersensitivity to paracetamol or any component of the suppository
- •Severe hepatic impairment or active liver disease
Drug Interactions
Alcohol
Concurrent use, especially chronic alcohol consumption, increases the risk of hepatotoxicity.
Warfarin
May increase the anticoagulant effect, leading to an increased risk of bleeding, especially with prolonged, high-dose paracetamol use. Monitor INR.
Chloramphenicol
May increase plasma concentration of chloramphenicol.
Phenytoin, Carbamazepine, Barbiturates
May increase the risk of paracetamol hepatotoxicity due to enzyme induction.
Storage
Store in a cool, dry place, below 25°C. Do not freeze. Protect from light and moisture.
Overdose
Overdose can cause severe, potentially fatal liver damage. Symptoms include nausea, vomiting, abdominal pain, loss of appetite, and general malaise, which may not appear until 24-48 hours after ingestion. Treatment involves immediate gastric lavage/activated charcoal if oral, and administration of N-acetylcysteine (NAC) as an antidote.
Pregnancy & Lactation
Considered safe for use during pregnancy and breastfeeding at recommended doses, as it is one of the most commonly used and best-studied analgesics/antipyretics in these populations. Always consult a doctor before use.
