Description
Overview of the medicine
Paracetamol (Acetaminophen) is a widely used over-the-counter analgesic and antipyretic. It is used for the relief of fever, headaches, and other minor aches and pains. The suppository form is particularly useful for patients who cannot take oral medication, such as children or those experiencing nausea and vomiting.
Uses & Indications
Dosage
Adults
500-1000 mg rectally every 4-6 hours as needed. Maximum 4000 mg (8 suppositories) in 24 hours.
Elderly
Similar to adult dosage, but caution with liver or renal impairment.
Renal_impairment
Adjust dose interval in severe renal impairment (e.g., extend to 8 hours for CrCl < 10 mL/min).
How to Take
For rectal use only. Insert deeply into the rectum. Wash hands thoroughly before and after use.
Mechanism of Action
Paracetamol is thought to act primarily in the central nervous system, inhibiting prostaglandin synthesis through a central mechanism. It also has weak anti-inflammatory effects. Its antipyretic action is due to effects on the hypothalamic heat-regulating center, causing peripheral vasodilation and increased heat loss.

Primarily renal (urine) as glucuronide and sulfate conjugates.

Absorption
Rapid and complete absorption from the rectal route, though peak plasma concentrations may be slightly delayed compared to oral administration.
Metabolism
Primarily hepatic, mainly by conjugation with glucuronide and sulfate. A small amount is metabolized by the cytochrome P450 system to a reactive intermediate (NAPQI), which is detoxified by conjugation with glutathione.
Side Effects
Contraindications
- •Hypersensitivity to paracetamol or any component of the suppository.

- •Recent rectal surgery or inflammation of the rectum/anus.
Drug Interactions
Alcohol
Increased risk of hepatotoxicity with chronic alcohol use.
Warfarin
May enhance anticoagulant effect with prolonged regular use, increasing risk of bleeding.
Phenobarbital, Phenytoin, Carbamazepine
May increase metabolism of paracetamol, reducing its effectiveness and increasing risk of hepatotoxicity.
Storage
Store below 30°C in a dry place, away from light. Do not freeze.
Overdose
Overdose can cause severe, sometimes fatal, liver damage. Symptoms may include nausea, vomiting, abdominal pain, and diaphoresis, appearing 24-48 hours after ingestion. Seek immediate medical attention. Treatment involves activated charcoal (if recent oral ingestion) and N-acetylcysteine.
Pregnancy & Lactation
Generally considered safe for occasional use at recommended doses during pregnancy and lactation. Consult a doctor before use.
