Description
Overview of the medicine
Napa 120 mg Syrup contains Paracetamol, which is used for rapid relief of fever, mild to moderate pain, headache, toothache, earache, body ache, myalgia, neuralgia, and period pain. It is also effective for pain associated with colds and flu.
Uses & Indications
Dosage
Adults
Not typically recommended for adults in syrup form due to high volume needed for effective dose. Adults usually take 500-1000 mg (e.g., 20-40 ml of 120mg/5ml syrup) every 4-6 hours, not exceeding 4000 mg in 24 hours.
Elderly
Similar to adult dosage, but with caution; consider reduced doses if hepatic or renal impairment is present.
Renal_impairment
Dose interval may need to be extended. Consult a physician for specific recommendations based on creatinine clearance.
How to Take
For oral administration. Can be taken with or without food. Use a measuring spoon or cup for accurate dosing.
Mechanism of Action
Paracetamol exerts its analgesic and antipyretic effects by inhibiting prostaglandin synthesis, primarily in the central nervous system (CNS). It reduces fever by direct action on the heat-regulating centers in the hypothalamus, leading to peripheral vasodilation and increased heat loss.

Onset
Analgesic effect starts within 30-60 minutes, antipyretic effect within 30 minutes.
Excretion
Excreted mainly by the kidneys as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
Plasma elimination half-life ranges from 1.5 to 3 hours.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract following oral administration. Peak plasma concentrations usually occur within 30 to 60 minutes.
Metabolism
Primarily metabolized in the liver by conjugation with glucuronic acid and sulfate. A small amount is metabolized by cytochrome P450 into a potentially toxic intermediate metabolite (NAPQI).
Side Effects
Contraindications

Drug Interactions
Alcohol
Increased risk of hepatotoxicity, especially with chronic heavy alcohol consumption.
Cholestyramine
Reduces the absorption of paracetamol, should be taken at least one hour apart.
Warfarin and other coumarins
May increase the anticoagulant effect, leading to an increased risk of bleeding. Regular monitoring of INR is advised.
Metoclopramide and Domperidone
May increase the absorption rate of paracetamol.
Other paracetamol-containing products
Increased risk of overdose and liver damage.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Overdose can cause severe, potentially fatal, liver damage. Symptoms may include nausea, vomiting, abdominal pain, and loss of appetite. Immediate medical attention is crucial. Treatment includes gastric lavage, activated charcoal, and administration of the antidote N-acetylcysteine (NAC) as soon as possible.
Pregnancy & Lactation
Paracetamol is generally considered safe for use during pregnancy and lactation at therapeutic doses. However, always consult a doctor before taking any medication during these periods.
