A-Pak 100 mg Tablet is a broad-spectrum antibiotic used to treat a wide range of bacterial infections. It belongs to the penicillin-like class of antibiotics and works by stopping the growth of bacteria. It is effective against both Gram-positive and Gram-negative bacteria.
Uses & Indications
Dosage
Adults
100 mg to 250 mg every 8 hours, depending on the severity of the infection. Maximum dose 500 mg every 8 hours.
Elderly
No specific dose adjustment is generally required unless there is significant renal impairment. Monitor renal function.
Renal_impairment
For creatinine clearance 10-30 mL/min: 100 mg every 12 hours. For creatinine clearance <10 mL/min: 100 mg every 24 hours.
How to Take
Take A-Pak 100 mg Tablet orally with a glass of water, with or without food. Complete the full course of treatment as prescribed by your doctor, even if symptoms improve.
Mechanism of Action
Apakcillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial lysis and death. It is bactericidal against susceptible organisms.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 1-2 hours after oral administration.
Excretion
Primarily excreted unchanged by the kidneys (glomerular filtration and tubular secretion); about 60-70% of the dose is excreted within 6 hours.
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract; bioavailability is approximately 80%. Food may slightly decrease the rate but not the extent of absorption.
Metabolism
Partially metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- Known hypersensitivity to apakcillin, other penicillins, or any component of the formulation.
- History of severe allergic reactions (e.g., anaphylaxis) to any beta-lactam antibiotic.
Drug Interactions
Probenecid
May increase and prolong plasma levels of apakcillin by decreasing renal tubular secretion.
Methotrexate
May increase methotrexate toxicity due to decreased renal clearance.
Oral contraceptives
May reduce the effectiveness of oral contraceptives (rarely).
Warfarin and other oral anticoagulants
May increase the anticoagulant effect, requiring closer monitoring of INR.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptoms may include nausea, vomiting, diarrhea, and rarely, seizures. Treatment is symptomatic and supportive. Hemodialysis may be used to remove apakcillin from the circulation in severe cases.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and the potential benefits outweigh the potential risks. Excreted in breast milk in small amounts; use with caution in breastfeeding mothers.
