Description
Overview of the medicine
Ace 120 mg Suspension is an oral medication containing Paracetamol (Acetaminophen), primarily used to relieve mild to moderate pain and reduce fever in children. It works by blocking pain signals and reducing heat in the body.
Uses & Indications
Dosage
Children
10-15 mg/kg body weight, every 4-6 hours as needed. Do not exceed 60 mg/kg/day or 4 doses in 24 hours. For Ace 120 mg/5 ml Suspension, consult packaging for specific age/weight-based dosing.
Renal_impairment
Dose interval may need to be extended. For CrCl 10-50 mL/min, administer every 6 hours. For CrCl <10 mL/min, administer every 8 hours. Consult a doctor.
Infants_under_3_months
Consult a doctor. Dose typically 10 mg/kg (for post-immunization fever only), or 5-10 mg/kg every 6-8 hours for other conditions under medical supervision.
How to Take
For oral administration. Shake the bottle well before use. Use the provided measuring spoon or cup for accurate dosing. Can be taken with or without food.
Mechanism of Action
Paracetamol is thought to primarily act by inhibiting prostaglandin synthesis in the central nervous system, which contributes to its analgesic (pain-relieving) and antipyretic (fever-reducing) effects. Its mechanism on peripheral tissues is less pronounced.
Pharmacokinetics
Onset
Analgesic and antipyretic effects typically begin within 30-60 minutes.
Excretion
Mainly excreted in the urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
Plasma elimination half-life is approximately 1 to 4 hours in adults; slightly shorter in children.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 30 to 60 minutes.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation. A small portion is metabolized by cytochrome P450 enzymes to a toxic intermediate (NAPQI), which is normally detoxified by conjugation with glutathione.
Side Effects
Contraindications
- Hypersensitivity to paracetamol or any other component of the formulation.
- Severe hepatic impairment or active liver disease.
Drug Interactions
Alcohol
Chronic alcohol consumption increases the risk of hepatotoxicity.
Cholestyramine
Reduces absorption of paracetamol if given within one hour.
Warfarin and other coumarin anticoagulants
May increase anticoagulant effect, leading to increased risk of bleeding. Monitor INR.
Enzyme-inducing drugs (e.g., phenobarbital, phenytoin, carbamazepine, rifampicin)
May increase the risk of paracetamol-induced hepatotoxicity by increasing the formation of toxic metabolites.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Do not freeze. Keep out of reach of children.
Overdose
Overdose of paracetamol can lead to severe, potentially fatal, hepatic necrosis. Symptoms include nausea, vomiting, abdominal pain, and pallor, which may appear 24-48 hours after ingestion. Treatment involves gastric lavage, activated charcoal, and administration of N-acetylcysteine (NAC) as an antidote.
Pregnancy & Lactation
Paracetamol is generally considered safe for use during pregnancy and lactation at recommended therapeutic doses. However, consult a healthcare professional before use, especially during pregnancy.

Lab Monitoring
- Liver function tests (ALT, AST) if overdose is suspected or with long-term high-dose therapy.
- Renal function tests if long-term high-dose use or in patients with pre-existing renal impairment.
Doctor Notes
- Emphasize precise dosing for pediatric patients using appropriate measuring devices.
- Educate parents about the risks of exceeding the recommended dose and the importance of avoiding concomitant use with other paracetamol-containing products.
- Counsel on early recognition of overdose symptoms and immediate medical intervention.
Patient Guidelines
- Always follow the prescribed dose and frequency; do not exceed the recommended daily dose.
- Do not use with other paracetamol-containing products to avoid overdose.
- If symptoms persist for more than 3 days (fever) or 5 days (pain) in children, consult a doctor.
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for the next dose. In that case, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Paracetamol is not known to affect the ability to drive or operate machinery.
Lifestyle Advice
- Ensure adequate hydration, especially when treating fever.
- Avoid alcohol consumption, especially during prolonged use of paracetamol, to reduce the risk of liver damage.
