Overview of the medicine
Aceclofenac is a non-steroidal anti-inflammatory drug (NSAID) used for the relief of pain and inflammation in conditions like osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis.
Uses & Indications
Dosage
Adults
100 mg twice daily, preferably with or after food.
Elderly
No specific dosage adjustment is usually necessary, but caution should be exercised due to potential renal or hepatic impairment and concomitant medication use. The lowest effective dose should be used.
Renal_impairment
For mild to moderate renal impairment, no dosage adjustment is typically needed, but monitor patients carefully. Avoid in severe renal impairment.
Hepatic_impairment
For mild hepatic impairment, an initial daily dose of 100 mg may be sufficient. Use with caution and monitor liver function. Avoid in severe hepatic impairment.
How to Take
Administer orally. It is recommended to take the tablet with or after food to minimize gastrointestinal discomfort.
Mechanism of Action
Aceclofenac acts primarily by inhibiting cyclooxygenase (COX) enzymes, particularly COX-2, which are responsible for the synthesis of prostaglandins, mediators of pain and inflammation.

Onset
Pain relief may begin within approximately 1 hour.
Excretion
Approximately two-thirds of the dose is excreted in the urine, mainly as hydroxylated metabolites, and about one-third via the faeces.
Half life
Approximately 4-4.3 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached in approximately 1.25 to 3 hours.
Metabolism
Extensively metabolized in the liver, primarily via CYP2C9, to its main metabolite, 4'-hydroxyaceclofenac.
Side Effects
Contraindications
- Hypersensitivity to aceclofenac or any other NSAID (e.g., aspirin, ibuprofen)
- Active gastrointestinal bleeding or peptic ulcer
- Severe renal or hepatic impairment
- Severe heart failure
- Third trimester of pregnancy
- Patients in whom aspirin or other NSAIDs precipitate attacks of asthma, urticaria, or acute rhinitis
Drug Interactions
Digoxin
May increase plasma digoxin levels.
Lithium
May increase plasma lithium levels, requiring dosage adjustment.
Methotrexate
Increased plasma levels of methotrexate and enhanced toxicity.
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding.
Diuretics and ACE inhibitors
May reduce the natriuretic effect of diuretics and the antihypertensive effect of ACE inhibitors, and increase the risk of renal impairment.
Anticoagulants (e.g., Warfarin)
Increased risk of gastrointestinal bleeding.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include headache, nausea, vomiting, epigastric pain, gastrointestinal bleeding, rarely diarrhea, disorientation, excitation, coma, drowsiness, dizziness, tinnitus, fainting, and convulsions. Management is primarily symptomatic and supportive, including gastric lavage and activated charcoal.
Pregnancy & Lactation
Pregnancy Category C in first and second trimesters, D in third trimester. Avoid in the third trimester due to potential adverse effects on the fetal cardiovascular system (premature closure of the ductus arteriosus). Use with caution during lactation; small amounts may pass into breast milk.
