Acefra 125 mg Suspension is an oral antibiotic belonging to the third-generation cephalosporin class. It contains Cefixime as its active ingredient and is used to treat a wide range of bacterial infections in children.
Uses & Indications
Dosage
Adults
Adults typically use higher strength tablets (e.g., 200-400 mg daily). This suspension is generally for pediatric use.
Elderly
Dosage adjustment is not generally required unless there is severe renal impairment.
Children
8 mg/kg/day administered as a single dose or in two divided doses every 12 hours for 5 to 10 days. Max 400 mg/day.
Renal_impairment
For creatinine clearance 21-60 mL/min, 75% of the standard dose should be given. For creatinine clearance <20 mL/min, 50% of the standard dose should be given.
How to Take
Shake the bottle well before each use. Administer orally with a measuring spoon or cup. Can be taken with or without food.
Mechanism of Action
Cefixime acts by inhibiting bacterial cell wall synthesis, leading to bacterial cell lysis and death. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains essential for bacterial cell wall integrity.

Onset
Peak plasma concentrations are achieved within 2-6 hours after administration.
Excretion
Primarily excreted unchanged in the urine (about 50-60% of the absorbed dose) and some biliary excretion.
Half life
The plasma elimination half-life is typically 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food may delay absorption but does not significantly affect the extent of absorption.
Metabolism
Cefixime is not extensively metabolized in the body; about 10% may undergo hepatic metabolism.
Side Effects
Contraindications
- Known hypersensitivity to Cefixime, other cephalosporins, or penicillins.
Drug Interactions
Warfarin
Increased prothrombin time (INR) and bleeding risk. Close monitoring of coagulation parameters is recommended.
Carbamazepine
Increased plasma concentrations of Carbamazepine when co-administered with Cefixime.
Oral Anticoagulants
Enhanced anticoagulant effect.
Storage
Store the dry powder below 30°C in a dry place. Protect from light. Store the reconstituted suspension in a refrigerator (2-8°C). Do not freeze.
Overdose
Gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Supportive treatment is recommended.
